Özet

Diyabetik ayak enfeksiyonları ve osteomiyelit tanısında nükleer tıp uygulamaları, anatomik ve fonksiyonel verileri birleştirerek klinisyenlere kritik bilgiler sunmaktadır. Hastalığın gelişiminde vaskülopati, nöropati ve bağışıklık sistemi yanıtındaki defektler temel patolojik süreçleri oluştururken, yumuşak doku enfeksiyonunun osteomiyelitten erken evrede ayırt edilmesi ampütasyon riskini azaltmak adına hayati önem taşır. Tanısal süreçte ilk seçenek ucuz ve yaygın olan direkt grafidir; ancak erken evre osteomiyeliti dışlamada yetersiz kaldığından, yüksek uzaysal çözünürlük sunan manyetik rezonans görüntüleme (MRG) güçlü bir adım olarak öne çıkar. Nükleer tıp yöntemlerinden üç fazlı kemik sintigrafisi yüksek duyarlılığa sahip olsa da, kırık ve Charcot nöroartropatisi gibi durumlarda düşük özgüllük gösterir. Bu sınırı aşmak adına, osteomiyelit tanısında altın standart kabul edilen in-vitro ve in-vivo işaretli lökosit sintigrafileri ile hibrit SPECT/BT protokolleri tercih edilmektedir. Son yıllarda, daha kısa sürede yüksek çözünürlüklü görüntü sağlayan 18F-FDG PET/BT ve PET/MR yöntemleri de önem kazanmıştır. Charcot artropatisi ile enfeksiyon ayrımında kemik iliği sintigrafisi kombinasyonu destekleyici rol oynarken, kesin tanı ve en doğru tedavi stratejisi için multimodalite görüntüleme tekniklerinin ve multidisipliner bir yaklaşımın bir arada kullanılması zorunludur.

Nuclear medicine applications in the diagnosis of diabetic foot infections and osteomyelitis provide critical information to clinicians by combining anatomic and functional data. While vasculopathy, neuropathy, and defects in immune response constitute the main pathological processes, distinguishing soft tissue infection from osteomyelitis at an early stage is vital to reduce amputation risks. Plain radiography is the initial choice due to availability, but since it is insufficient in ruling out early osteomyelitis, magnetic resonance imaging (MRI) with high spatial resolution stands out as a strong subsequent step. Among nuclear medicine methods, three-phase bone scintigraphy has high sensitivity but shows low specificity in conditions like fractures and Charcot neuroarthropathy. To overcome this limitation, labeled leukocyte scintigraphy, considered the gold standard, and hybrid SPECT/CT protocols are preferred. Recently, 18F-FDG PET/CT and PET/MR methods, providing high-resolution images in shorter times, have gained importance. While bone marrow scintigraphy helps differentiate Charcot arthropathy from infection, multi-modality imaging and a multidisciplinary approach are mandatory for accurate diagnosis and optimal treatment planning.
