Özet

Diyabet, serbest radikallerin artışı, oksidatif stres ve nonenzimatik glikozilasyon gibi mekanizmalarla karakterize kronik bir hastalıktır. Bu süreçler hücresel hasara ve çeşitli komplikasyonlara yol açtığından, antioksidan savunma sistemini destekleyen vitaminlerin rolü kritik önem taşır. Tedavide yaygın olarak kullanılan metforminin, doz ve süreye bağlı olarak makrositer anemi ve nöropatiye yol açabilen B12 vitamini eksikliğine neden olduğu bilinmektedir. Suda eriyen güçlü bir antioksidan olan C vitamini, insülin direncini azaltmaya ve HbA1c düzeylerini düşürmeye yardımcı olurken, eksikliğinde doku hasarı ve yara iyileşmesinde gecikme gözlenir. Steroid yapılı bir prohormon olan D vitamini ise insülin salınımı ve glikoz metabolizmasında işlev görür; eksikliği prediyabetten diyabete geçişi hızlandırabilir. Hücre membranlarını koruyan lipid itrahlı E vitamini ile doku farklılaşmasında rol oynayan A vitamini de antioksidan kapasiteyi artırarak komplikasyonları önlemede etkilidir. Sonuç olarak, diyabet yönetiminde HbA1c takibi, sağlıklı beslenme ve yaşam tarzı değişiklikleri esastır; klinik bir eksiklik saptanmadıkça dışarıdan rutin vitamin takviyesi önerilmemektedir.

Diabetes is a chronic disease characterized by increased free radicals, oxidative stress, and nonenzymatic glycosylation. Since these processes lead to cellular damage and various complications, the role of vitamins supporting the antioxidant defense system is of critical importance. Metformin, widely used in treatment, is known to cause vitamin B12 deficiency depending on the dose and duration, which can lead to macrocytic anemia and neuropathy. Vitamin C, a potent water-soluble antioxidant, helps reduce insulin resistance and lower HbA1c levels, while its deficiency causes tissue damage and delayed wound healing. Vitamin D, a steroid prohormone, functions in insulin release and glucose metabolism; its deficiency can accelerate the transition from prediabetes to diabetes. Vitamin E, which protects cell membranes, and vitamin A, which plays a role in tissue differentiation, are also effective in preventing complications by increasing antioxidant capacity. Consequently, monitoring HbA1c, healthy nutrition, and lifestyle changes are essential in diabetes management; routine vitamin supplementation is not recommended unless a clinical deficiency is detected.
