Çocuk ve Adolesanlarda Diyabetik Böbrek Hastalığı

Özet

Diabetes mellitus (DM), mikrovasküler komplikasyonları ile yaşam kalitesini ciddi ölçüde düşüren yaygın bir metabolik bozukluktur. Bu komplikasyonlardan en sık görüleni olan diyabetik böbrek hastalığı (DBH), mikroalbüminüri (MA) ile başlayıp son dönem böbrek yetmezliğine (SDBY) kadar ilerleyebilen sinsi bir süreçtir. Çocuk ve ergenlerde daha nadir görülse de, tip 2 DM'li gençlerde tip 1'e kıyasla daha erken ve agresif seyretmektedir. Hastalığın tanısı temelde albümin atılımı ve glomerüler filtrasyon hızındaki (GFR) değişimlere dayanır; spot idrarda albümin/kreatinin oranının taranması erken teşhis için kritik öneme sahiptir. DBH patogenezinde kalıcı hiperglisemi, gelişmiş glikasyon son ürünleri, oksidatif stres, vasküler hasar ve renin-anjiyotensin sisteminin aktivasyonu rol oynar. Hastalığın ilerlemesini durdurmak ve profilaksi sağlamak amacıyla, HbA1c hedeflerinin (<%7,0-7,5) sıkı yönetildiği glisemik kontrol, yaşam tarzı değişiklikleri ve kan basıncının kontrol altında tutulması elzemdir. Hipertansiyon ve proteinüri tedavisinde ACE inhibitörleri ile anjiyotensin reseptör blokerleri (ARB) ilk basamak farmakoterapi olarak tercih edilmektedir. Ayrıca dislipidemi yönetimi ve protein kısıtlaması içeren diyet müdahaleleri de böbrek fonksiyonlarının korunmasında önemli katkılar sunmaktadır.

Diabetes mellitus (DM) is a common metabolic disorder that severely reduces the quality of life through microvascular complications. Diabetic kidney disease (DKD), the most frequent of these complications, is an insidious process that begins with microalbuminuria (MA) and can progress to end-stage renal disease (ESRD). Although rarer in children and adolescents, it manifests earlier and progresses more aggressively in youth with type 2 DM compared to type 1 DM. Diagnosis relies primarily on changes in albumin excretion and glomerular filtration rate (GFR); screening the random spot urine albumin-to-creatinine ratio is critical for early detection. The pathogenesis of DKD involves persistent hyperglycemia, advanced glycation end-products, oxidative stress, vascular injury, and activation of the renin-angiotensin system. To halt disease progression and provide prophylaxis, strict glycemic control targeting specific HbA1c levels (<7.0-7.5%), lifestyle modifications, and blood pressure management are essential. ACE inhibitors and angiotensin receptor blockers (ARBs) are preferred as first-line pharmacotherapy for treating hypertension and proteinuria. Furthermore, dyslipidemia management and dietary interventions involving protein restriction provide significant contributions to preserving renal functions.
