Özet

Radyodermatit, radyoterapi alan kanser hastalarının yaklaşık %95’inde görülen, tedavi dozuna, uygulama alanına ve genetik yatkınlığa bağlı olarak değişen derecelerde kutanöz reaksiyonlarla seyreden en yaygın yan etkilerden biridir. Klinik olarak, tedaviyi takip eden ilk 90 günde eritem, ödem, kıl dökülmesi ve deskuamasyon ile karakterize "akut" evre; aylar veya yıllar sonra ortaya çıkan atrofi, telenjiektazi ve ilerleyici subkutan fibrozis ile karakterize "kronik" evre olmak üzere iki kronolojik süreçte incelenir. Akut evrede bazal keratinositlerin hasarı ve inflamatuar sitokinlerin artışı patogenezde rol oynarken, kronik dönemdeki fibrotik değişiklikleri esas olarak fibroblast aktivasyonunu uyaran TGF-beta sitokini yönetir. Önleme ve tedavi stratejilerinde; IMRT ve VMAT gibi modern radyoterapi tekniklerinin kullanımı, seans öncesi kalın tabakalı uygulamalardan kaçınılması şartıyla profilaktik topikal kortikosteroidler ve hassas deri bakımı ilkeleri öne çıkar. Reaksiyonun şiddeti NCI CTCAE veya RTOG/EORTC sistemlerine göre derecelendirilerek; hafif olgularda nemlendiriciler, nemli deskuamasyonda özel pansumanlar, nekroz gelişen ileri evrelerde ise cerrahi debridman ve greftleme gibi basamaklı tedaviler uygulanır. Ayrıca süreçte, kemoterapötiklerle tetiklenen radyasyon recall fenomeni ve sekonder kutanöz malignite riskleri de yakından izlenmelidir.

Radiodermatitis is one of the most frequent adverse effects of radiotherapy, affecting approximately 95% of cancer patients and presenting with cutaneous reactions that vary based on radiation dose, treated volume, and genetic susceptibility. Chronologically, it is evaluated in two phases: the "acute" stage, occurring within the first 90 days of treatment and characterized by erythema, edema, alopecia, and desquamation; and the "chronic" stage, which manifests months or years later with atrophy, telangiectasia, and progressive subcutaneous fibrosis. While basal keratinocyte destruction and inflammatory cytokine upregulation drive acute pathogenesis, the chronic fibrotic alterations are predominantly mediated by TGF-beta, which stimulates permanent fibroblast activation. Prevention and management strategies emphasize advanced radiotherapy techniques like IMRT and VMAT, meticulous skin care protocols, and prophylactic topical corticosteroids, provided they are not applied in thick layers shortly before irradiation sessions. Severity is graded using NCI CTCAE or RTOG/EORTC criteria, guiding a tiered management approach that ranges from hydrophilic moisturizers for mild cases to specialized wound dressings for moist desquamation, and surgical debridement or grafting for full-thickness necrosis. Additionally, clinicians must monitor secondary risks, including chemotherapy-induced radiation recall phenomena and late-onset secondary cutaneous malignancies.
