Omuz Eklemi Muayenesi ve Klinik Değerlendirilmesi
Özet
Omuz eklemi şikayetleri, ortopedi polikliniklerinde dizden sonra ikinci sıklıkla görülür ve en yaygın nedenleri subakromiyal sıkışma sendromu ile rotator manşet patolojileridir. Sistematik bir omuz muayenesi; detaylı hasta hikayesi, fizik muayene ve görüntüleme yöntemlerinin birleşimiyle doğru tanıya ulaşılmasını sağlar. Hikaye aşamasında hastanın yaşı, mesleği, spor geçmişi, dominant eli ve ağrının karakteri (gece ağrısı, baş üstü aktiviteler) ön tanı için kritik ipuçları sunar. Fizik muayene ise inspeksiyon, palpasyon, aktif-pasif eklem hareket açıklığı (ROM) ölçümleri ve spesifik provokatif testlerle devam eder. Klinikte supraspinatus sıkışmasını değerlendirmek için Neer, Hawkins ve Ağrılı Ark testleri kullanılırken; biceps patolojileri için Speed, Yergason ve Lippman testlerinden yararlanılır. İnstabilite durumlarında anterior endişe, relokasyon ve sulkus bulgusu test edilirken; labrum hasarları için O'brein, rotator manşet yırtıkları için ise düşük kol testi uygulanır. Son olarak, omuz ağrısını taklit edebilecek C5-6 radikülopatisi gibi servikal sorunları dışlamak amacıyla kapsamlı bir nörolojik muayene gerçekleştirilmelidir.
Shoulder complaints are the second most common issues in orthopedics clinics after knee complaints, with subacromial impingement syndrome and rotator cuff pathologies being the most frequent causes. A systematic shoulder evaluation ensures an accurate diagnosis through a combination of detailed patient history, physical examination, and imaging methods. During the history-taking phase, the patient's age, occupation, sports history, dominant hand, and pain characteristics (night pain, overhead activities) provide critical clues for a preliminary diagnosis. The physical examination continues with inspection, palpation, active-passive range of motion (ROM) measurements, and specific provocative tests. In clinical practice, Neer, Hawkins, and Painful Arc tests are used to evaluate supraspinatus impingement, while Speed, Yergason, and Lippman tests assess biceps pathologies. For instability, anterior apprehension, relocation, and sulcus sign are tested; for labral lesions, the O'brien test is utilized, and for rotator cuff tears, the drop arm test is performed. Finally, a comprehensive neurological examination must be conducted to rule out cervical issues, such as C5-6 radiculopathy, which can mimic shoulder pain.
