Kalsifik Tendinopatiler
Özet
Kalsifik tendinopati (KT), tendonlarda (özellikle rotator manşette) hidroksiapatit kalsiyum birikmesiyle karakterize, kendi kendini sınırlayan bir hastalıktır. Etiyolojisinde hipovaskülarizasyon, metabolik faktörler, travma ve enfeksiyonlar yer almaktadır. Hastalık; prekalsifikasyon, kalsifikasyon ve postkalsifikasyon olmak üzere üç klinik/histolojik aşamada gelişir. Yetişkinlerin %3'ünde görülen KT, ağrılı omuz semptomlarının %40-54'ünün nedenidir ve sıklıkla 40-50 yaş arası çalışan kadınlarda görülür. Tipik klinik belirtisi, kısıtlı hareket açıklığı ile geceleri artan subakut omuz ağrısıdır; kalsifikasyonlar genellikle birkaç yıl içinde kendiliğinden rezorbe olur. Tanıda standart direkt grafiler en sık kullanılan yöntem olup, manşet yırtıklarıyla ayırıcı tanıda ultrasonografi ve MRG da tercih edilir. Tedavinin temelini istirahat, fizik tedavi ve NSAİİ'leri içeren konservatif yöntemler oluşturur. Akut fazda ultrason eşliğinde lokal anesteziyle iğne aspirasyonu etkilidir; ancak kronik fazda kalsiyum katılaştığı için etkinliği sınırlıdır. Diğer yöntemler arasında kalsifikasyonu şok dalgalarıyla parçalayan ESWT ve konservatif tedaviye yanıt vermeyen hastalarda tercih edilen cerrahi (açık veya sıklıkla artroskopik) yaklaşımlar yer alır; artroskopi daha az invaziv olması ve subakromiyal dekompresyona izin vermesi nedeniyle avantajlıdır.
Calcific tendinopathy (CT) is a self-limiting disease characterized by hydroxyapatite calcium accumulation in tendons, particularly in the rotator cuff. Hypovascularization, metabolic factors, trauma, and infections are implicated in its etiology. The disease progresses through three clinical/histological stages: precalcification, calcification, and postcalcification. Occurring in approximately 3% of adults, CT causes 40-54% of painful shoulder symptoms and is more common among working women aged 40-50. The typical clinical symptom is subacute shoulder pain that increases at night with a restricted range of motion; calcium deposits usually spontaneously resorb within a few years. Standard direct radiographs are the most frequently used method for diagnosis, while ultrasonography and MRI are preferred for differential diagnosis from cuff tears. The cornerstone of treatment consists of conservative methods including rest, physical therapy, and NSAIDs. In the acute phase, needle aspiration under ultrasound guidance with local anesthesia is effective, but its efficacy is limited in the chronic phase as the calcium hardens. Other methods include ESWT, which fragments calcification using shock waves, and surgical approaches (open or frequently arthroscopic) preferred for patients unresponsive to conservative treatments; arthroscopy is advantageous due to being less invasive and allowing subacromial decompression.
