Özet

Nörolojik muayene, hasta ile hekim arasındaki işbirliğine dayanan; özellikle dört yaş altı çocuklarda gözlemin kritik öneme sahip olduğu kapsamlı bir değerlendirme sürecidir. Muayene, bilincin Glasgow Koma Skalası ile değerlendirilmesiyle başlar. Ardından görme, koku, göz hareketleri ve yüz kaslarını kontrol eden on iki kraniyal sinir sistematik olarak incelenir. Motor sistem muayenesinde kas gücü Medical Research Council skalasına göre derecelendirilirken; traksiyon ve süspansiyon manevralarıyla kas tonusu, gözlemle ise kas kitlesi analiz edilir. Derin tendon refleksleri ve Babinski gibi patolojik reflekslerin varlığı, piramidal sistemin durumunu ortaya koyar. Ekstrapiramidal sistem değerlendirmesi; kore, distoni ve tremor gibi istem dışı hareketlerin tespitine odaklanır. Duyu muayenesi, yüzeyel ve derin duyular ile parietal lob fonksiyonlarını gösteren yüksek kortikal duyuları kapsar. Serebellar sistem muayenesinde ise ardışık hareketler ve denge test edilir. Son aşamada hastanın yürüyüşü ve postürü gözlemlenerek spastik, ataksik veya stepaj gibi yürüyüş bozuklukları tanımlanır; ense sertliği ve Kernig gibi meninks irritasyon bulguları kontrol edilerek süreç tamamlanır.

Neurological examination is a comprehensive evaluation process based on cooperation between patient and physician, where observation is of critical importance, especially in children under four years of age. The examination begins with the assessment of consciousness using the Glasgow Coma Scale. Subsequently, twelve cranial nerves controlling vision, smell, eye movements, and facial muscles are systematically examined. In the motor system examination, muscle strength is graded according to the Medical Research Council scale; muscle tone is analyzed with traction and suspension maneuvers, and muscle mass is evaluated through observation. The presence of deep tendon reflexes and pathological reflexes such as Babinski reveals the state of the pyramidal system. Extrapyramidal system evaluation focuses on the detection of involuntary movements such as chorea, dystonia, and tremor. Sensory examination covers superficial and deep sensations, as well as high cortical sensations indicating parietal lobe functions. In the cerebellar system examination, sequential movements and balance are tested. In the final stage, the patient's gait and posture are observed to define gait disturbances such as spastic, ataxic, or steppage; the process is completed by checking for meningeal irritation signs such as neck stiffness and Kernig.
