Yenidoğan Muayenesi
Özet
Yenidoğan muayenesi, yaşamın ilk 28 gününü kapsayan ve bebeklerin sağlıklı gelişimini izlemek amacıyla yapılan sistematik bir süreçtir. Muayene öncesinde bebeğin doğum haftası ve ağırlığına göre sınıflandırılması (prematüre, term, SGA, AGA, LGA) kritik önem taşır. Değerlendirme süreci; aile öyküsünü içeren prenatal, doğum anını kapsayan natal ve doğum sonrasını kapsayan postnatal olmak üzere üç aşamalı bir hikaye alımıyla başlar. Fizik muayene; uygun çevre koşullarında, bebeğin genel görünümü, vücut ölçüleri (kilo, boy, baş çevresi) ve vital bulgularının (ateş, solunum, kalp hızı) ölçülmesiyle gerçekleştirilir. Sistemik muayenede deri bulguları, fontanellerin durumu, kafa yapısındaki doğum travması izleri (kaput suksadenum, sefal hematom) ve yüz simetrisi dikkatle incelenir. Göz, kulak, burun ve ağız muayenelerinde konjenital anomalilerin tespiti hedeflenir. Kardiyovasküler ve solunum sistemlerinde kalp sesleri, üfürümler, solunum paterni ve femoral nabızlar değerlendirilirken, karın muayenesinde organ büyüklükleri ve umbilikal kord incelenir. Genital bölge, anüs açıklığı ve kalça displazisi (Ortolani ve Barlow testleri) kontrolleri yapıldıktan sonra nörolojik muayeneye geçilir. Nörolojik değerlendirmede kas tonusu ve Moro, yakalama, emme gibi ilkel reflekslerin varlığı, merkezi sinir sistemi sağlığının en önemli göstergeleridir.
Newborn examination is a systematic process conducted during the first 28 days of life to monitor the healthy development of infants. Before the examination, classifying the baby based on gestational age and weight (premature, term, SGA, AGA, LGA) is of critical importance. The evaluation starts with a three-stage history taking: prenatal (family history), natal (birth details), and postnatal (post-birth issues). Physical examination is performed under suitable environmental conditions by observing the general appearance and measuring body parameters (weight, length, head circumference) and vital signs (temperature, respiration, heart rate). In the systemic examination, skin findings, the state of fontanelles, signs of birth trauma on the head (caput succedaneum, cephalohematoma), and facial symmetry are carefully inspected. Examinations of the eyes, ears, nose, and mouth aim to detect congenital anomalies. In the cardiovascular and respiratory systems, heart sounds, murmurs, respiratory patterns, and femoral pulses are evaluated, while the abdomen is checked for organ size and the umbilical cord. After checking the genitalia, anal patency, and hip dysplasia (Ortolani and Barlow tests), the neurological exam is conducted. In the neurological evaluation, muscle tone and the presence of primitive reflexes such as Moro, grasp, and sucking are the most important indicators of central nervous system health.
