Normal Pubertal Gelişim
Özet
Puberte, çocukluktan genç erişkinliğe geçişi temsil eden, ikincil cinsiyet karakterlerinin geliştiği ve üreme yeteneğinin kazanıldığı karmaşık bir biyolojik süreçtir. Bu süreç, hipotalamustan salınan gonadotropin salgılatıcı hormonun (GnRH) pulsatil salınımıyla hipotalamus-hipofiz-gonad (HHG) aksının aktive olmasıyla başlar. HHG aksı fetal, neonatal (mini-puberte) ve pubertal olmak üzere yaşamın üç evresinde aktiftir. Pubertenin başlama zamanı genetik etmenler başta olmak üzere beslenme, genel sağlık durumu ve çevresel faktörlerden etkilenir; özellikle leptin hormonu vücut ağırlığı ve yağ dokusu ile beyin arasındaki iletişimi sağlayarak süreci tetikleyici rol oynar. Normal puberte başlangıcı kızlarda 8-13 yaş, erkeklerde ise 9-14 yaş aralığında gerçekleşir. Kızlarda ilk bulgu meme gelişimi (telarş) iken, erkeklerde testis hacminin 4 ml ve üzerine çıkmasıdır. Gelişim evreleri Tanner-Marshall ölçeği ile beş basamakta takip edilir. Kızlarda büyüme atağı pubertenin erken evrelerinde (evre 2-3) görülürken, erkeklerde daha geç (evre 3-4) ve daha belirgin şekilde yaşanır. Türk çocuklarında ortalama menarş yaşı 12,2 olarak saptanmıştır. Belirlenen yaş sınırlarından önce başlayan veya belirtilen sürede gerçekleşmeyen gelişimler erken veya gecikmiş puberte olarak tanımlanır.
Puberty is a complex biological process representing the transition from childhood to young adulthood, during which secondary sexual characteristics develop and reproductive capacity is gained. This process begins with the activation of the hypothalamus-pituitary-gonadal (HPG) axis through the pulsatile release of gonadotropin-releasing hormone (GnRH) from the hypothalamus. The HPG axis is active during three stages of life: fetal, neonatal (mini-puberty), and pubertal. The onset of puberty is influenced by genetic factors, nutrition, general health status, and environmental factors; specifically, the hormone leptin plays a triggering role by mediating communication between body weight/adipose tissue and the brain. The normal onset of puberty occurs between ages 8-13 in girls and 9-14 in boys. The first clinical sign in girls is breast development (thelarche), whereas in boys, it is an increase in testicular volume to 4 ml or more. Developmental stages are monitored in five steps using the Tanner-Marshall scale. While the growth spurt occurs in the early stages of puberty (stages 2-3) for girls, it occurs later (stages 3-4) and more significantly for boys. The average age of menarche in Turkish children has been determined as 12.2 years. Development starting before the specified age limits or not occurring within the expected timeframe is defined as precocious or delayed puberty.
