Ağrılı Kraniyal Nöropatiler ve Diğer Yüz Ağrıları
Özet
Uluslararası Baş Ağrısı Bozuklukları Sınıflandırması’nın 13. grubunda yer alan ağrılı kraniyal nöropatiler, çocukluk döneminde nadir görülmekle birlikte pediatrik nörolojide tanısal zorluklar oluşturabilen klinik tablolardır. Trigeminal nevralji bu grubun en sık karşılaşılan formu olup, genellikle karbamazepin gibi antiepileptiklerle tedavi edilse de dirençli vakalarda cerrahi girişimler gerekebilir. Tolosa-Hunt sendromu, ağrılı oftalmoplejik nöropati ve okipital nevralji gibi diğer türler, altta yatan sistemik veya vasküler hastalıkların dışlanmasını gerektiren karmaşık ayırıcı tanı süreçlerine sahiptir. Doğru tedavi protokollerinin belirlenmesi, etiyolojinin netleşmesi ve semptomların yaşam kalitesini iyileştirmesi açısından büyük önem taşır.
Painful cranial neuropathies, classified under group 13 of the International Classification of Headache Disorders, present diagnostic challenges in pediatric neurology despite their relative rarity in childhood. Trigeminal neuralgia is the most frequently encountered form, typically managed with antiepileptics like carbamazepine, though refractory cases may necessitate surgical intervention. Other conditions, such as Tolosa-Hunt syndrome, painful ophthalmoplegic neuropathy, and occipital neuralgia, require complex differential diagnoses to exclude underlying systemic or vascular pathologies. Establishing accurate treatment protocols is critical for identifying specific etiologies and improving the patient's quality of life.
