Psikiyatrik Bozukluklara Bağlı Baş Ağrısı
Özet
Başağrısı, çocuk ve ergenlerde okul başarısını ve sosyal işlevselliği ciddi oranda etkileyen, psikiyatrik bozukluklarla sıkça komorbidite gösteren önemli bir sağlık sorunudur. Psikiyatrik hastalıklar bazen başağrısının temel nedeni (sekonder başağrısı) olabildiği gibi, bazen de birincil başağrılarının şiddetlenmesine veya kronikleşmesine yol açan tetikleyici bir faktör olarak rol oynamaktadır. Tanı sürecinde biyobelirteçlerden ziyade detaylı hasta öyküsü ve klinik değerlendirme öne çıkmakta, tedavi yönetiminde ise çocuk ruh sağlığı uzmanlarıyla iş birliği içinde multidisipliner bir yaklaşım ile bilişsel davranışçı terapiler ve farmakolojik yöntemler tercih edilmektedir. Somatizasyon bozuklukları, depresyon, anksiyete ve travma sonrası stres bozukluğu gibi durumlar başağrısı ile sık birliktelik göstererek klinik tablonun yönetimini daha kompleks hale getirmektedir.
Headache is a significant health issue in children and adolescents that severely impacts academic performance and social functionality, frequently showing comorbidity with psychiatric disorders. Psychiatric illnesses can sometimes be the primary cause of headaches (secondary headache), or they can act as a triggering factor leading to the exacerbation or chronicization of primary headaches. During the diagnostic process, detailed patient history and clinical assessment are prioritized over biomarkers, and treatment management requires a multidisciplinary approach in collaboration with child mental health specialists, often involving cognitive-behavioral therapies and pharmacological methods. Conditions such as somatization disorders, depression, anxiety, and post-traumatic stress disorder often coexist with headaches, making the management of the clinical picture more complex.
