Otoimmün Hastalıklar ve Baş Ağrısı
Özet
Çocuk ve ergenlerde sıkça görülen baş ağrıları genellikle primer olsa da, vaskülitler, SLE, MS ve çeşitli romatolojik hastalıklar gibi otoimmün süreçler vasküler veya parankimal tutulumlarla sekonder baş ağrılarına yol açabilmektedir. Şimşekvari başlangıç, uykudan uyandırma veya sistemik bulguların eşlik etmesi gibi "alarm işaretleri", bu ciddi tabloların primer baş ağrılarından ayırt edilmesinde hayati öneme sahiptir. Tanıda detaylı öykü ve fizik muayenenin yanı sıra spesifik otoantikor testleri ve nörogörüntüleme yöntemleri kullanılmalı, altta yatan sistemik bozukluklar titizlikle araştırılmalıdır. Baş ağrısı, otoimmün hastalıklarda bazen ilk klinik belirti bazen de tromboz veya infarkt gibi ciddi komplikasyonların habercisi olabileceğinden, vakalar sistematik bir yaklaşımla ele alınmalıdır.
While headaches in children and adolescents are predominantly primary, autoimmune and rheumatological conditions—including vasculitis, SLE, and MS—often induce secondary headaches through vascular involvement, inflammation, or treatment side effects. Identifying "alarm signs" such as sudden onset, nocturnal occurrence, or association with systemic symptoms is crucial for differentiating these potentially serious pathologies from primary headache disorders. Effective diagnosis relies on a comprehensive history, thorough physical examination, specific autoantibody testing, and neuroimaging to investigate underlying systemic involvement. Since headaches can manifest as either the initial sign or an indicator of severe complications like thrombosis in autoimmune diseases, systematic evaluation of all patients is essential.
