Status Migrenozus

Özet

Status migrenozus, 72 saatten uzun süren, auralı veya aurasız migrenin şiddetli bir komplikasyonudur. Tanısı ICHD-3 kriterlerine dayanır ve altta yatan patofizyolojisinde trigeminovasküler sistem ile kalsitonin geni ile ilişkili peptit (CGRP) gibi nörolojik faktörler rol oynar. Tedavi yaklaşımı; antidopaminerjikler, triptanlar, NSAID'ler ve IV sıvı replasmanı gibi kombinasyon terapilerini içerirken, dirençli vakalarda ketamin, propofol veya lidokain gibi ileri seçenekler değerlendirilebilir. Eğitim ve tetikleyicilerin yönetimi, bu durumun önlenmesinde kritik rol oynar.

 

Status migrenosus is a severe complication of migraine defined as an attack lasting more than 72 hours. Diagnosis is based on ICHD-3 criteria, and its pathophysiology involves neurological factors such as the trigeminovascular system and calcitonin gene-related peptide (CGRP). Treatment strategies include combination therapies like antidopaminergics, triptans, NSAIDs, and IV fluid resuscitation, while refractory cases may require advanced options such as ketamine, propofol, or lidocaine. Effective prevention relies on patient education and the management of individual triggers.
