Periyodik Paraliziler

Özet

Periyodik paraliziler, ağır egzersiz, oruç veya karbonhidrat ağırlıklı beslenme gibi faktörlerle tetiklenen, ağrısız kas güçsüzlüğü atakları ile karakterize nadir bir nöromüsküler bozukluk grubudur. Serum potasyum seviyelerine göre hipokalemik ve hiperkalemik olarak sınıflandırılan bu hastalıklar, genellikle genetik kökenli olup iskelet kası kanalopati ailesinde yer alırlar. Ataklar sırasında hastaların bilinci korunurken, solunum ve bulbar kaslar genellikle etkilenmez, ancak tirotoksikoz gibi ikincil nedenler veya farklı sendromlarla ayırıcı tanısının yapılması kritik öneme sahiptir. Tedavide yaşam tarzı düzenlemeleri, potasyum takviyeleri ve belirli diüretik veya karbonik anhidraz inhibitörleri gibi farmakolojik yöntemler tercih edilmektedir.

 

Periodic paralyses are a group of rare neuromuscular disorders characterized by episodes of painless muscle weakness, often triggered by intense exercise, fasting, or high-carbohydrate meals. Classified based on serum potassium levels into hypokalemic and hyperkalemic types, these conditions are primarily genetic channelopathies, though secondary causes such as thyrotoxicosis can also precipitate attacks. During episodes, patients maintain consciousness and typically experience minimal involvement of bulbar or respiratory muscles, though differential diagnosis is essential to distinguish these from other systemic causes. Management strategies include lifestyle modifications, potassium supplementation, and pharmacological interventions such as diuretics or carbonic anhydrase inhibitors.
