Transvers Miyelit
Özet
Transvers miyelit, spinal kordun bağışıklık mekanizmalarıyla tetiklenen, akut veya subakut başlangıçlı motor ve duyu kayıpları ile sfinkter disfonksiyonuna yol açan demiyelinizan bir hastalıktır. Tanı, spinal inflamasyon bulgularının varlığı ve diğer benzer durumların dışlanması ile konulmakta olup radyolojik görüntüler ve BOS incelemeleri kritik rol oynamaktadır. Tedavide öncelikli olarak yüksek doz steroid kullanımı tercih edilirken, dirençli vakalarda plazma değişimi veya immünglobülin gibi ek yöntemler uygulanmaktadır. Hastalık süreci genellikle monofazik seyretse de prognoz, hastanın klinik bulgularına ve tedaviye yanıtına göre değişiklik gösterebilmektedir.
Transverse myelitis is an immune-mediated demyelinating disease of the spinal cord characterized by the acute or subacute onset of sensory and motor deficits along with sphincter dysfunction. Diagnosis relies on confirming spinal cord inflammation while excluding other potential conditions through spinal imaging and cerebrospinal fluid analysis. Management typically involves high-dose steroid therapy, with plasma exchange or other immunosuppressive treatments reserved for resistant cases. Although the condition is often monophasic, patient outcomes vary significantly based on the severity of symptoms and the timeliness of clinical intervention.
