Özet
Progresif multifokal lökoensefalopati (PML), genellikle immünsüpresyon varlığında JC virüsünün (JCV) reaktivasyonu sonucu ortaya çıkan, merkezi sinir sistemini etkileyen ölümcül bir hastalıktır. Virüs, beyindeki myelin üreten oligodendrositlerde litik enfeksiyona ve buna bağlı olarak beyaz cevherde multifokal demyelinizasyona yol açar. Klinik tablo, bilişsel gerileme, motor bozukluklar ve nörolojik kayıplarla karakterizedir. Spesifik bir antiviral tedavisi bulunmamakta olup, en etkili strateji immün sistemin yeniden yapılandırılmasıdır.
Progressive multifocal leukoencephalopathy (PML) is a fatal disease of the central nervous system caused by the reactivation of the JC virus (JCV), typically occurring in the presence of immunosuppression. The virus causes lytic infection of oligodendrocytes, leading to multifocal demyelination within the brain's white matter. Clinical presentation is characterized by cognitive decline, motor impairment, and various neurological deficits. There is no specific antiviral treatment, and immune reconstitution remains the most effective strategy for management.
