İntihar Davranışını Açıklayan İkinci Nesil Kuramlar
Özet
2000’li yıllardan sonra ortaya çıkan ve intihar düşüncesinin gelişimi ile intihar eylemini birbirinden ayıran "düşünceden eyleme geçiş" odaklı ikinci nesil çağdaş psikolojik intihar kuramları kapsamlı bir şekilde incelenmektedir. Bölümde öne çıkan dört temel model; Kişilerarası İntihar Kuramı (KAİK), Üç Aşamalı İntihar Kuramı, Bütünleyici Güdüsel-İradesel Kuram (BGİ) ve Değişken Yatkınlık Kuramı (DYK) olarak sıralanmaktadır. KAİK, intihar arzusunu aidiyet hissinin engellenmesi ve başkalarına yük olma algısı temelinde açıklarken, ölümcül eylem için fiziksel acıya tolerans gibi sonradan kazanılmış intihar yeteneğini şart koşmaktadır. Üç Aşamalı İntihar Kuramı, sürecin acı ve umutsuzluğun birleşimiyle başladığını, bağlılığın koruyuculuğu ile şekillendiğini ve pratik yeteneklerle eyleme dönüştüğünü savunmaktadır. BGİ kuramı ise diyatez-stres modeline dayanarak intiharı güdülenme öncesi, güdülenme ve irade olmak üzere üç aşamada ele almakta; yenilgi ve kapana kısılma hissinin intihar bilişlerini tetiklediğini iradesel faktörlerin ise eylemi kolaylaştırdığını belirtmektedir. Son olarak DYK, intihar riskinin gün içinde dalgalanan doğrusal olmayan, dinamik yapısına odaklanarak riski temel ve akut boyutlarda sınıflandırıp bilişsel inanç sistemlerinin dönüştürülmesinin tedavi sürecindeki kritik önemini vurgulamaktadır.
Second-generation contemporary psychological suicide theories focused on the "ideation-to-action" framework that emerged after the 2000s, which differentiate the development of suicidal ideation from the suicidal act itself, are comprehensively examined. The four main models highlighted in the section are listed as the Interpersonal Theory of Suicide (IPTS), the Three-Step Theory (3ST), the Integrated Motivational-Volitional (IMV) Model, and the Fluid Vulnerability Theory (FVT). While IPTS explains suicidal desire based on thwarted belongingness and perceived burdensomeness, it requires an acquired capability for suicide, such as physical pain tolerance, for the fatal act to occur. The Three-Step Theory posits that the process begins with the combination of pain and hopelessness, is shaped by the protective nature of connectedness, and transforms into action through practical capabilities. The IMV model, based on the diathesis-stress framework, addresses suicide in three phases as pre-motivational, motivational, and volitional, stating that feelings of defeat and entrapment trigger suicidal cognitions, while volitional factors facilitate the act. Lastly, FVT focuses on the non-linear, dynamic nature of suicide risk that fluctuates throughout the day, classifying risk into stable and acute dimensions, and underscores the critical importance of transforming cognitive belief systems in the treatment process.
