Psikiyatrik Bozukluklarda İntihar ve Farmakolojik Tedavisi
Özet
Bu metin, psikiyatrik bozukluklarda intihar riskini ve farmakolojik tedavi yaklaşımlarını kapsamlı bir şekilde incelemektedir. Psikiyatride bir acil olarak kabul edilen intihar davranışı, genel popülasyona kıyasla ruhsal hastalığı olan bireylerde 3 ila 12 kat daha yüksek oranda görülmektedir. İntiharla en sık ilişkilendirilen durumlar major depresif bozukluk (MDB), bipolar bozukluk, şizofreni, alkol/madde kullanım bozuklukları, kişilik bozuklukları, anksiyete ve uyku bozukluklarıdır. Çalışmalar, MDB ve bipolar bozukluk gibi duygudurum bozukluklarının en yüksek risk grubunu oluşturduğunu; MDB'de ketamin tedavisinin hızlı ve antisuisidal etkiler sunduğunu, bipolar bozuklukta ise lityumun intihar riskini beş kat azalttığını göstermektedir. Şizofrenide intihar, özellikle hastalığın ilk yılında ve akut dönemlerde önde gelen ölüm nedenidir ve klozapin kullanımının bu riski azalttığı saptanmıştır. Anksiyete bozuklukları içinde en güçlü ilişki travma sonrası stres bozukluğunda görülürken; B kümesi kişilik bozukluklarından borderline ve antisosyal kişilik bozukluklarında dürtüsellik ve kendine zarar verme eğilimi öne çıkmaktadır. Ayrıca, yeme bozukluklarından anoreksiya nervozada intihar en sık ikinci ölüm nedeniyken, insomnianın da depresyondan bağımsız olarak intihar riskini önemli ölçüde artırdığı saptanmıştır. Sonuç olarak, intihar davranışını önlemek için klinisyenlerin çok yönlü risk faktörlerini saptaması ve hasta merkezli multidisipliner yaklaşımlar geliştirmesi kritik önem taşımaktadır.
This text comprehensively examines suicide risk and pharmacological treatment approaches in psychiatric disorders. Mental illness increases the risk of suicide 3 to 12 times compared to the general population. Mood disorders, such as Major Depressive Disorder (MDD) and bipolar disorder, carry the highest risk; ketamin offers rapid anti-suicidal effects in MDD, while lithium reduces suicide risk fivefold in bipolar disorder. In schizophrenia, suicide is a leading cause of death, particularly in the first year, and clozapine has been shown to mitigate this risk. Among anxiety disorders, post-traumatic stress disorder shows the strongest link to suicide, whereas among personality disorders, borderline and antisocial personality disorders stand out due to impulsivity and self-harm. Additionally, suicide is the second leading cause of death in anorexia nervosa, and insomnia independently increases suicide risk significantly. Consequently, identifying multifaceted risk factors and implementing patient-centered, multidisciplinary approaches are critical for clinicians to effectively prevent suicidal behavior.
