İntiharda Psikanalitik Yönelimli Psikoterapiler
Özet
İntihar davranışı, özellikle genç ve ileri yaş nüfus ile orta yaş erkeklerde küresel çapta önemli bir halk sağlığı sorunudur. Bu alanda psikanalitik yönelimli psikoterapilerin kanıta dayalı olmadığına dair yanlış bir kanı olsa da son yıllarda yapılan araştırmalar, bu terapilerin depresyon, anksiyete ve intihar girişimlerini anlamlı derecede azalttığını kanıtlamıştır. İntihar, sadece psikiyatrik bir bozukluğun belirtisi olmayıp, kişinin iç dünyasındaki derin umutsuzluk, çatışma ve çaresizlik hislerinin şiddetli bir dışa vurumu olarak ele alınmalıdır. İntihar eğilimi olan bireylerde genellikle yaşama devam etme isteği ile çaresizlik arasında ambivalantik (çelişkili) bir tutum gözlenir. Psikodinamik arka planda ego regresyonu, narsisistik krizler, kendilik değerini düzenleme kapasitesindeki eksiklikler ve aleksitimi gibi kişilik özellikleri ile inkar ve yansıtmalı özdeşim gibi savunma mekanizmaları rol oynamaktadır. Bu riske sahip bireylerle yürütülen terapilerde, güçlü bir terapötik işbirliği kurmak, aktarım ve karşı aktarım süreçlerini derinlemesine çalışmak merkezi bir öneme sahiptir; terapistin hastanın iç dünyasını anlamaya çalışırken esnek ve dengeli bir aktif tutum sergilemesi hayati bir gerekliliktir.
Suicidal behavior is a major global public health issue, particularly among the youth, the elderly, and middle-aged men. Despite the misconception that psychoanalytically oriented psychotherapies lack an empirical basis, recent studies have demonstrated that these therapies significantly reduce depression, anxiety, and suicide attempts. Suicide should not be viewed merely as a symptom of a psychiatric disorder, but rather as a severe manifestation of inner conflicts, profound hopelessness, and helplessness within an individual's internal world. Individuals with suicidal tendencies often exhibit an ambivalent attitude, torn between the desire to sustain life and feelings of helplessness. From a psychodynamic perspective, ego regression, narcissistic crises, structural deficits in regulating self-esteem, alexithymia, and defense mechanisms such as denial and projective identification play crucial roles. In psychotherapies conducted with these individuals, establishing a robust therapeutic alliance and thoroughly analyzing transference and countertransference processes are centrally important; it is vital for the therapist to maintain a flexible and well-balanced active stance while striving to comprehend the patient's inner world.
