Özet

Blefaroplasti ve lateral kantoplasti operasyonlarını ele alan bu metinde, alt göz kapağının anatomik yapısı, cerrahi yaklaşımları ve lateral kantoplasti uygulamaları detaylandırılmaktadır. Fonksiyonel veya kozmetik amaçlarla yapılan bleferoplasti, göz kapağındaki fazla dokuların uzaklaştırılması ve şekillendirilmesi esasına dayanır. Alt göz kapağı anatomik olarak anterior, orta ve posterior olmak üzere üç lamellaya ayrılmaktadır. Yaşlanmaya bağlı olarak orbital septumun zayıflaması sonucu orbital yağ dokusunun fıtıklaşması, tipik göz altı torbalarına ve yorgun bir görünüme yol açar. Bu deformitelerin düzeltilmesinde transkutanöz ve transkonjonktival olmak üzere iki ana cerrahi teknik tercih edilmektedir; transkonjonktival yaklaşım daha ziyade cilt fazlalığı olmayan genç hastalarda skar oluşumunu önlemek amacıyla uygulanır. Diğer taraftan lateral kantoplasti, yaşlanma, güneşe maruz kalma veya genetik faktörler nedeniyle gevşeyen ve sarkan alt göz kapaklarını yeniden konumlandırmak ve kantal eğimi düzeltmek amacıyla gerçekleştirilen bir prosedürdür. Cerrahi esnasında lateral kantal bölgedeki yüzeyel ligament diseke edilerek lateral tars serbest bırakılır ve periosta sütüre edilerek sağlam bir tespit sağlanır. Bleferoplasti ile kombine edilebilen bu işlemler, alt göz kapağının anatomik devamlılığını ve estetik görünümünü restore etmeyi hedefler.

Blepharoplasty and lateral canthoplasty operations are detailed in this text, covering the anatomical structure, surgical approaches, and applications of lateral canthoplasty for the lower eyelid. Blepharoplasty, performed for functional or cosmetic reasons, is based on the excision and reshaping of excess tissues in the eyelid. The lower eyelid is anatomically divided into three lamellae: anterior, middle, and posterior. With aging, the weakening of the orbital septum leads to the herniation of orbital fat tissue, causing typical under-eye bags and a tired appearance. Two main surgical techniques, transcutaneous and transconjunctival, are preferred to correct these deformities; the transconjunctival approach is chosen primarily in younger patients without skin excess to prevent scar formation. On the other hand, lateral canthoplasty is a procedure performed to reposition the sagging lower eyelids caused by aging, sun exposure, or genetic factors, and to correct the canthal tilt. During surgery, the superficial ligament in the lateral canthal region is dissected, the lateral tarsus is freed, and it is sutured to the periosteum to ensure secure fixation. These procedures, which can be combined with blepharoplasty, aim to restore the anatomical continuity and aesthetic appearance of the lower eyelid.
