Terapötik İlaç İzlemi

Özet

Terapötik ilaç izlemi (Tİİ), dar terapötik aralığa sahip ilaçların çeşitli biyolojik sıvılardaki konsantrasyonlarının ölçülüp yorumlanmasıyla, tedavide optimum etkinliği sağlarken toksisiteyi en aza indirmeyi amaçlayan kritik bir sağlık hizmetidir. Özellikle fizyolojileri ve ilaç kinetikleri sürekli değişkenlik gösteren yoğun bakım hastalarında, dozların bireyselleştirilmesini ve etkinliğin artırılmasını sağlar. Tİİ'nin başarılı olabilmesi için ilaçların kararlı duruma (Css) ulaşma süreleri, uygulama yolları, hastanın böbrek/karaciğer fonksiyonları ve doğru örnek alma zamanlaması (vadi veya pik düzeyleri) gibi biyolojik faktörlerin titizlikle göz önünde bulundurulması gerekir. Yoğun bakım ünitelerinde yaygın olarak kullanılan vankomisin, aminoglikozitler, digoksin, fenitoin, valproat ve teofilin gibi ilaçların her birinin kendine özgü hedef terapötik aralıkları ve örnekleme protokolleri bulunmaktadır. Yapılan çalışmalar, doğru zamanlama ve klinik parametrelerle uygulanan Tİİ rehberliğindeki tedavilerin, hastaların maruz kaldığı advers etkileri belirgin şekilde azalttığını, klinik sonuçları iyileştirdiğini ve hastaneler için maliyet etkililiği sağladığını açıkça ortaya koymaktadır.

Therapeutic drug monitoring (TDM) is a critical healthcare service that aims to ensure optimum efficacy while minimizing toxicity in treatment by measuring and interpreting the concentrations of narrow therapeutic index drugs in various biological fluids. It enables the individualization of doses and enhances efficacy, particularly in intensive care patients whose physiology and drug kinetics constantly fluctuate. For TDM to be successful, biological factors such as the time required for drugs to reach steady-state (Css), administration routes, patient's renal/hepatic functions, and precise sampling timing (trough or peak levels) must be meticulously considered. Commonly used drugs in intensive care units, including vancomycin, aminoglycosides, digoxin, phenytoin, valproate, and theophylline, each possess distinct target therapeutic ranges and sampling protocols. Studies clearly demonstrate that TDM-guided therapies, when implemented with correct timing and clinical parameters, significantly reduce adverse events, improve clinical outcomes, and provide cost-effectiveness for hospitals.
