Özet

Hipertansiyon, sürekli kan basıncı yüksekliğiyle seyreden ve ciddi komplikasyonlara yol açabilen yaygın bir sistemik hastalıktır. Tedavide temel amaç kan basıncını aniden düşürmek değil, hedef organ hasarını ve buna bağlı riskleri azaltmaktır. Tedavi yaklaşımları parenteral ve oral ilaçlar olarak iki ana sınıfta gruplandırılır. Akut hedef organ hasarının eşlik ettiği hipertansif acillerde, kan basıncını hızlı ve kontrollü şekilde düşürmek amacıyla yoğun bakım şartlarında intravenöz (iv) parenteral ilaçlar (nitrogliserin, nitroprussid, klevidipin, nikardipin vb.) tercih edilir. Kronik hipertansiyon tedavisinde ise kalsiyum kanal blokerleri, ACE inhibitörleri, ARB'ler, diüretikler ve beta blokerler gibi oral ajanlar kullanılır. İlaç seçimi; hastanın yaşına, ırkına ve kalp yetmezliği, diyabet veya böbrek hastalığı gibi eşlik eden komorbiditelerine göre bireyselleştirilir. Kontrolsüz hipertansiyon, semptom göstermeden ilerleyebildiği için "sessiz katil" olarak adlandırılmakta olup miyokard enfarktüsü, felç ve böbrek yetmezliği gibi ölümcül sonuçlar doğurabilmektedir.

Hypertension is a common systemic disease characterized by persistently elevated blood pressure that can lead to serious complications. The primary goal of treatment is not merely lowering blood pressure, but preventing complications and reducing overall risk. Therapeutic approaches are grouped into two main categories as parenteral and oral drugs. In hypertensive emergencies accompanied by acute target organ damage, intravenous parenteral agents (such as nitroglycerin, nitroprusside, clevidipine, and nicardipine) are preferred under intensive care conditions to lower blood pressure rapidly and controllably. For chronic hypertension treatment, oral agents such as calcium channel blockers, ACE inhibitors, ARBs, diuretics, and beta-blockers are utilized. Drug selection is individualized based on factors including the patient's age, race, and comorbidities like heart failure, diabetes, or renal disease. Uncontrolled hypertension, which often progresses without noticeable symptoms and is thus termed a "silent killer," can result in fatal consequences such as myocardial infarction, stroke, and renal failure.
