Nöroprotektif İlaçlar

Özet

Bu makale, kalp durması, vasküler olaylar veya kafa travması gibi nedenlerle gelişen hipoksik iskemik beyin hasarı ve buna bağlı bilinç bozukluklarının (koma, bitkisel durum ve minimal bilinçlilik) tedavisinde kullanılan nöroprotektif ilaçları ele almaktadır. Bilincin geri kazanılması sürecinde kullanılan farmakolojik ajanlar; santral sinir sistemi (SSS) depresanları ve SSS stimülanları olmak üzere iki ana kategoride gruplandırılmaktadır. SSS depresanlarından zolpidem uyanıklığı geçici olarak artırırken, baklofen spastisiteyi azaltıp omurga-korteks iletimini düzenleyerek, lamotrijin ise özellikle bilişsel performansı destekleyerek fonksiyonel iyileşmeyi teşvik edebilir. SSS stimülanları arasında yer alan dopaminerjik ajanlardan levodopa, amantadin, bromokriptin, apomorfin ve pramipeksol; hasar gören dopaminerjik yolakları uyararak uyanıklık, motor kontrol ve kognitif fonksiyonlarda düzelmeyi hızlandırır. Özellikle amantadin, travmatik beyin hasarı sonrası fonksiyonel iyileşmeyi hızlandırmada en çok çalışılan ve güvenle kullanılan ajandır. Diğer stimülanlardan metilfenidat ve amfetamin dikkat ile öğrenme hızını artırırken, antidepresanlar (SSRI ve trisiklikler) ile modafinil bilişsel, nöropsikiyatrik eksiklikleri ve gündüz uykululuğunu iyileştirir. Sonuç olarak, bilinç bozukluklarının tedavisinde henüz optimal bir tedavi kılavuzu oluşturacak yeterli kanıt bulunmamakla birlikte, her vakanın kendi içinde bütüncül ele alınması ve birleşik yaklaşımlara odaklanan klinik araştırmaların yapılması önerilmektedir.

This article discusses neuroprotective drugs used in the treatment of hypoxic-ischemic brain injury resulting from causes such as cardiac arrest, vascular events, or head trauma, and the associated disorders of consciousness (coma, vegetative state, and minimally conscious state). The pharmacological agents used in the recovery of consciousness are grouped into two main categories: central nervous system (CNS) depressants and CNS stimulants. Among the CNS depressants, zolpidem temporarily increases arousal, baclofen provides improvement by reducing spasticity and regulating spinal-to-cortical transmission, and lamotrigine promotes functional recovery by specifically supporting cognitive performance. Among the CNS stimulants, dopaminergic agents such as levodopa, amantadine, bromocriptine, apomorphine, and pramipexole accelerate recovery in arousal, motor control, and cognitive functions by stimulating damaged dopaminergic pathways. In particular, amantadine is the most studied and safely used agent to accelerate functional recovery after traumatic brain injury. Other stimulants like methylphenidate and amphetamine enhance attention and learning speed, while antidepressants (SSRIs and tricyclics) and modafinil improve cognitive and neuropsychiatric deficits as well as daytime sleepiness. Consequently, although there is not yet sufficient evidence to propose an optimal treatment guideline for disorders of consciousness, it is recommended to manage each case holistically within itself and to conduct clinical research focused on combined approaches.
