Özet

Çocuklarda ve bebeklerde fokal bir enfeksiyon odağı saptanmayan ateş (LBOA) vakalarına yaklaşımı ele alan bu metin, hastanın yaşına ve klinik görünümüne göre değişen tanı ve tedavi protokollerini detaylandırmaktadırÖzellikle üç yaş altı çocuklarda daha sık görülen bu durumun yönetiminde; Rochester, Philadelphia ve Yale gibi kriterler kullanılarak ciddi bakteriyel enfeksiyon riskinin belirlenmesi, tam kan sayımı, idrar tetkiki ve gerekli durumlarda BOS incelemesi gibi laboratuvar süreçlerinin titizlikle yürütülmesi kritik önem taşımaktadırMetin, aşılamanın yaygınlaşmasıyla azalan bakteriyemi riskine rağmen yenidoğanların ve toksik görünümlü çocukların acil müdahale ile hastaneye yatırılmasını, iyi görünen büyük çocuklarda ise daha konservatif bir izlem stratejisinin benimsenmesini önermektedir.

Fever without a localized source in infants and children remains a significant clinical challenge, often requiring careful risk assessment and diagnostic testing to distinguish between self-limiting viral illnesses and serious bacterial infections. Management strategies are primarily guided by the patient's age and clinical appearance, utilizing established criteria like the Rochester, Philadelphia, and Yale scales to determine the risk of bacteremia or meningitis. While widespread immunization has reduced the incidence of certain bacterial infections, clinicians must still perform thorough evaluations, including laboratory tests and imaging when necessary, to ensure timely treatment or appropriate observation.
