Gastrointestinal Sistem Kanamalarına Yaklaşım
Özet
Çocukluk çağı gastrointestinal kanamaları (GİK), yenidoğandan adölesan döneme kadar farklı etiyolojik nedenlerle ortaya çıkabilen ve acil yaklaşım gerektiren önemli klinik tablolardır. Kanama nedenleri yaş gruplarına göre değişkenlik göstermekte; yenidoğanlarda sıklıkla K vitamini eksikliği ve besin alerjileri görülürken, daha büyük çocuklarda peptik ülserler, vaskülitler (HSP) ve yabancı cisim yutulması ön plana çıkmaktadır. Tanı ve tedavide hastanın hemodinamik stabilizasyonu ilk öncelik olup, sonrasında endoskopik girişimler hem odak tespiti hem de tedavi edici müdahale açısından altın standart olarak kabul edilmektedir.
Gastrointestinal bleeding (GIB) in childhood is a significant clinical condition that requires urgent intervention and can arise from various etiologies depending on the age group, from neonates to adolescents. Causes vary significantly by age: while vitamin K deficiency and food allergies are common in newborns, peptic ulcers, vasculitis (HSP), and foreign body ingestion are more prevalent in older children. Hemodynamic stabilization is the primary priority in diagnosis and management, followed by endoscopic procedures, which remain the gold standard for both identifying the source and providing therapeutic intervention.
