Çocuk ve Ergenlerde Uyku Bozukluklarının Tanımı ve Epidemiyolojisi
Özet
Çocuk ve ergenlerde uyku bozuklukları, %25-40 oranında görülen ve yaşam kalitesini, gelişimi ile ruh sağlığını derinden etkileyen yaygın bir sağlık problemidir. Uyku bozuklukları; uykusuzluk (insomnia), aşırı uykululuk (hipersomnolans), narkolepsi, solunumla ilişkili bozukluklar, sirkadiyen ritim bozuklukları ve parasomniler (uyurgezerlik, kâbus bozukluğu vb.) gibi geniş bir yelpazede ele alınmaktadır. Özellikle yaşamın ilk yıllarında gece uyanmaları ve uykuya dalma güçlükleri ön plandayken, yaş ilerledikçe okul reddi, anksiyete ve yetersiz uyku sendromu gibi sorunlar daha sık rapor edilmektedir. Tanı koyma sürecindeki en büyük zorluk uykunun yaş, kültür ve çevresel faktörlere göre değişkenlik göstermesidir. Araştırmalar, uyku problemlerinin dikkat eksikliği ve hiperaktivite bozukluğu (DEHB), otizm spektrum bozukluğu ve depresyon gibi psikiyatrik durumlarla çift yönlü ve karmaşık bir ilişki içinde olduğunu; bu sorunların tedavi edilmediği takdirde akademik başarıda düşüş, davranış sorunları ve ilerleyen yaşlarda ciddi ruhsal hastalık risklerini artırdığını göstermektedir. Sonuç olarak, çocukluk çağı uyku bozukluklarının erken tespiti ve multidisipliner bir yaklaşımla ele alınması, bireyin sağlıklı gelişimi açısından kritik bir öneme sahiptir.
Sleep disorders in children and adolescents are common health problems seen at rates of 25-40% that profoundly affect the quality of life, development, and mental health. These disorders are addressed in a broad spectrum, including insomnia, hypersomnolence, narcolepsy, breathing-related disorders, circadian rhythm disorders, and parasomnias such as sleepwalking and nightmare disorder. While night wakings and difficulties falling asleep are prominent in the first years of life, issues like school refusal, anxiety, and insufficient sleep syndrome are reported more frequently as age progresses. The greatest challenge in the diagnostic process is that sleep varies according to age, culture, and environmental factors. Research indicates that sleep problems have a complex, bidirectional relationship with psychiatric conditions such as ADHD, autism spectrum disorder, and depression; if left untreated, these problems increase the risk of decreased academic success, behavioral issues, and serious mental illnesses in later years. Consequently, early detection and a multidisciplinary approach to childhood sleep disorders are of critical importance for the healthy development of the individual.
