Çocuk ve Ergenlerde REM Uykusu Davranış Bozukluğu
Özet
REM uykusu davranış bozukluğu (RUDB), rüyaların fiziksel olarak canlandırılmasına yol açan REM atoni kaybı ile karakterize bir parasomnidir. İlk başlarda yaşlı erkeklere özgü olduğu düşünülse de, günümüzde çocuklarda ve ergenlerde de narkolepsi, antidepresan kullanımı ve nöropsikiyatrik bozukluklarla ilişkili olarak görüldüğü saptanmıştır. Bozukluğun patofizyolojisi beyin sapındaki inhibitör motor mekanizmaların, özellikle pons ve subcoeruleus kompleksinin işlev bozukluğu ile ilgilidir. Klinik olarak hastalar, rüya içeriğine bağlı olarak yumruklama, tekmeleme ve seslendirme gibi agresif davranışlar sergileyebilir ve bu durum hem hastanın hem de yatak partnerinin yaralanmasına neden olabilir. Tanı için altın standart, atonisiz REM uykusunu belgeleyen laboratuvar içi video polisomnografidir. Ayırıcı tanıda NREM parasomnileri, kâbuslar ve epilepsi gibi durumlar dikkatle elenmelidir. Tedavinin öncelikli amacı güvenli bir uyku ortamı sağlamaktır; farmakolojik olarak ise düşük doz klonazepam ve melatonin en etkili seçenekler olarak öne çıkmaktadır. Çocuk ve ergen popülasyonunda nadir görülmesi nedeniyle, bu yaş grubundaki etiyolojiyi tam olarak anlamak için daha fazla araştırmaya ihtiyaç duyulmaktadır.
REM sleep behavior disorder (RBD) is a parasomnia characterized by the loss of REM atonia, leading to the physical enactment of dreams, and while initially associated with elderly males, it is now recognized in children and adolescents, often linked to narcolepsy, antidepressant use, or neuropsychiatric disorders. The pathophysiology involves dysfunction in inhibitory brainstem motor mechanisms, particularly the pons and subcoeruleus complex, resulting in clinical manifestations such as punching, kicking, and vocalizations that can cause injury to the patient or their bed partner. Diagnosis requires in-laboratory video polysomnography to document REM sleep without atonia, while the differential diagnosis must carefully exclude NREM parasomnias, nightmares, and nocturnal seizures. The primary goal of treatment is establishing a safe sleep environment, with low-dose clonazepam and melatonin serving as the most effective pharmacological options. Given its rarity in the pediatric and adolescent population, further research is essential to fully understand the specific etiologies and long-term implications in these age groups.
