Çocuk ve Ergenlerde Duygudurum Bozuklukları ve Uyku
Özet
Çocuk ve ergenlerde duygudurum bozuklukları ile uyku arasındaki ilişkiyi inceleyen bu çalışma, uykunun fiziksel ve ruhsal sağlık üzerindeki hayati önemini vurgulayarak, uyku bozukluklarının klinik tabloyu karmaşıklaştırdığını ortaya koymaktadır. Araştırmalar, majör depresif bozukluğu olan gençlerin büyük bir kısmında insomnia veya hipersomnia görüldüğünü, bu sorunların depresyonun şiddetini artırdığını ve tedavi yanıtını olumsuz etkilediğini göstermektedir. Özellikle ergenlerde uyku problemleri ile intihar düşüncesi ve kendine zarar verme davranışı arasında güçlü bir bağ tespit edilmiştir. Pediatrik bipolar bozuklukta ise azalan uyku ihtiyacı manik dönemlerin ayırt edici bir belirtisi olarak öne çıkarken, uyku düzensizliklerinin duygudurum ataklarını tetikleyebildiği saptanmıştır. Tedavi tarafında, farmakolojik yöntemlerin yanı sıra uykusuzluk için uygulanan bilişsel davranışçı terapinin (BDT-I), hem uyku kalitesini hem de duygudurum semptomlarını iyileştirmede etkili olduğu belirtilmektedir. Sonuç olarak, uyku bozukluklarının erken teşhisi ve tedavisi, çocuk ve ergenlerde duygudurum bozukluklarının önlenmesi ve başarılı bir şekilde yönetilmesi için kritik bir müdahale hedefi olarak değerlendirilmektedir.
This study examining the relationship between mood disorders and sleep in children and adolescents emphasizes the vital importance of sleep for physical and mental health, revealing that sleep disorders complicate the clinical picture. Research shows that a large proportion of youth with major depressive disorder experience insomnia or hypersomnia, and these problems increase the severity of depression and negatively affect treatment response. A strong link has been identified between sleep problems and suicidal ideation and self-harming behavior, especially in adolescents. In pediatric bipolar disorder, decreased need for sleep stands out as a distinguishing symptom of manic episodes, while it has been found that sleep irregularities can trigger mood episodes. On the treatment side, in addition to pharmacological methods, cognitive behavioral therapy for insomnia (CBT-I) is stated to be effective in improving both sleep quality and mood symptoms. Consequently, early diagnosis and treatment of sleep disorders are considered a critical intervention target for the prevention and successful management of mood disorders in children and adolescents.
