Travma Sonrası Stres Bozukluğu (TSSB), hayati tehlike arz eden olaylar sonrası gelişen ve çocuklarla ergenlerde ciddi uyku bozukluklarına yol açan karmaşık bir psikiyatrik tablodur. Bu vakalarda insomnia, kâbuslar ve uyku yapısında bozulmalar sıkça görülmekte; bu sorunlar sadece bir semptom değil, hastalığı kronikleşdiren çekirdek unsurlar olarak kabul edilmektedir. TSSB tanılı çocuklarda uykusuzluk ve kâbus görme oranları, travmaya maruz kalmayan yaşıtlarına göre belirgin düzeyde yüksektir. Tedavi yaklaşımlarında yetişkin verileri baskın olsa da, insomnia için Bilişsel Davranışçı Terapi (BDT-i) ve kâbuslar için İmgelem Prova Terapisi (IRT) gibi yöntemler öne çıkmaktadır. Farmakolojik alanda çocuklara yönelik onaylı ilaç bulunmamakla birlikte, prazosin gibi ajanlar kâbusların şiddetini azaltmada kullanılmaktadır. Mevcut araştırmalar, TSSB tedavisinin uyku sorunlarını tamamen çözmediğini, bu nedenle uyku bozukluklarının birincil bir hedef olarak ele alınması gerektiğini vurgulamaktadır. Özellikle çocuk ve ergen popülasyonuna yönelik özelleşmiş tanı araçlarına ve daha kapsamlı çalışmalara ihtiyaç duyulmaktadır.

Post-Traumatic Stress Disorder (PTSD) is a complex psychiatric condition that develops after life-threatening events and leads to severe sleep disturbances in children and adolescents. In these cases, insomnia, nightmares, and disruptions in sleep architecture are frequently observed; these issues are now recognized not merely as secondary symptoms but as core features that contribute to the chronicity of the disorder. Rates of insomnia and nightmares in children diagnosed with PTSD are significantly higher than in their non-traumatized peers. Although treatment approaches are largely based on adult data, methods such as Cognitive Behavioral Therapy for Insomnia (CBT-i) and Imagery Rehearsal Therapy (IRT) for nightmares stand out. In the pharmacological field, while there are no approved medications specifically for children, agents like prazosin are used to reduce the severity of nightmares. Current research indicates that treating PTSD alone does not fully resolve sleep-related problems, thus sleep disturbances must be addressed as a primary clinical target. There is a critical need for specialized diagnostic tools and more comprehensive studies focusing specifically on the pediatric and adolescent population.
