Kronobiyolojik Yönüyle Dikkat Eksikliği Hiperaktivite Bozukluğu
Özet
Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB), çocuklukta başlayıp yetişkinlikte de devam edebilen heterojen ve nörogelişimsel bir psikiyatrik rahatsızlıktır. Bu bilimsel metinde, DEHB’nin temel semptomları olan dikkatsizlik, hiperaktivite ve dürtüsellik boyutlarının ötesine geçilerek, kronobiyolojik işlev bozuklukları, sirkadiyen ritim anormallikleri ve bunlara bağlı gelişen metabolik süreçler kapsamlıca incelenmektedir. İnsanlarda sirkadiyen sistem anterior hipotalamustaki suprakiazmatik nukleus tarafından yönetilmekte olup, uyku-uyanıklık, hormon salınımı ve beslenme gibi birçok yaşamsal ritmi organize eder. Araştırmalar, DEHB tanısı almış bireylerde sağlıklı kontrollere kıyasla yatağa gitme ve uykuya dalma güçlüğü, sık uyanma, huzursuz bacak sendromu ve uykuda solunum bozuklukları gibi niceliksel ve niteliksel uyku sorunlarının çok daha yüksek oranlarda görüldüğünü kanıtlamaktadır. Ayrıca çift yönlü bir etkileşime sahip olan sirkadiyen ritim ve beslenme mekanizmalarındaki bozulmalar neticesinde, DEHB popülasyonunda obezite sendromu, tıkınırcasına yeme ve bulimiya nervoza gibi yeme bozukluklarına yakalanma riskinin anlamlı ölçüde arttığı saptanmıştır. Bireylerin günlük performans eğilimlerini ifade eden kronotip açısından bakıldığında ise, DEHB'li çocuklarda çelişkili veriler bulunsa da, DEHB'li erişkinlerin genel popülasyona göre çok daha yüksek bir oranda "akşamcıl kronotip" sergilediği ve bu durumun epifiz bezi hacmi küçüklüğü ile saat genlerindeki polimorfizmlerle ilişkili olabileceği belirtilmektedir. Sonuç olarak DEHB, biyolojik saat moleküler anormalliklerini bünyesinde barındıran geniş bir kronobiyolojik bozukluk yelpazesi sunduğundan, gelecekte geliştirilecek kronoterapi ve kronofarmakoloji yaklaşımları tedavi başarısında kritik bir rol oynayacaktır.
Attention Deficit Hyperactivity Disorder (ADHD) is a heterogeneous and neurodevelopmental psychiatric condition that begins in childhood and persists into adulthood. In this scientific text, moving beyond the core symptoms of inattention, hyperactivity, and impulsivity, chronobiological dysfunctions, circadian rhythm abnormalities, and associated metabolic processes are comprehensively examined. The circadian system in humans is governed by the suprachiasmatic nucleus in the anterior hypothalamus, organizing many vital rhythms such as sleep-wake, hormone release, and nutrition. Research demonstrates that individuals diagnosed with ADHD experience much higher rates of quantitative and qualitative sleep problems compared to healthy controls, including difficulty going to bed and falling asleep, frequent awakenings, restless legs syndrome, and sleep-disordered breathing. Furthermore, as a result of disruptions in circadian rhythm and nutritional mechanisms, which possess a bidirectional interaction, a significantly increased risk of developing obesity syndrome and eating disorders like binge eating and bulimia nervosa has been identified in the ADHD population. Regarding chronotype, which expresses daily performance tendencies, although conflicting data exist for children with ADHD, adults with ADHD display a much higher rate of "evening chronotype" compared to the general population, which is thought to be associated with smaller pineal gland volume and clock gene polymorphisms. In conclusion, since ADHD presents a broad spectrum of chronobiological disorders harboring molecular abnormalities of the biological clock, future chronotherapy and chronopharmacology approaches will play a critical role in treatment success.
