Özet

Torakolomber geçiş sendromu (TLGS), torakal ve lomber vertebraların birleştiği bölgedeki (sıklıkla T12-L1) disfonksiyon nedeniyle oluşan ve bel ağrısının sıklıkla göz ardı edilen bir nedenidir. Robert Maigne tarafından tanımlanan bu sendromda ağrı, anatomik farklılıklar nedeniyle rotasyonel stresin bu bölgede yoğunlaşmasından kaynaklanır. Hastalar ağrıyı genellikle lumbosakral, inguinal veya gluteal bölgelerde hissederler; bu durum yanlış teşhislere ve gereksiz cerrahilere yol açabilir. Tanı; iliak krest nokta testi, deri yuvarlama testi ve spinal hassasiyetin değerlendirildiği klinik muayene ile konur. Tedavide manuel terapi ve manipülasyon oldukça etkilidir; dirençli vakalarda faset eklem enjeksiyonları veya radyofrekans uygulamaları tercih edilir. Biyomekaniğin düzeltilmesi için egzersiz ve postür eğitimi tedavinin ayrılmaz bir parçasıdır.

Thoracolumbar junction syndrome (TLJS) is an often-overlooked cause of low back pain, resulting from dysfunction at the transition between thoracic and lumbar vertebrae, most commonly at T12-L1. Defined by Robert Maigne, the pain in this syndrome stems from rotational stress concentrated in this region due to anatomical differences. Patients typically report pain in the lumbosacral, inguinal, or gluteal regions, which can lead to misdiagnosis and unnecessary surgeries. Diagnosis is established through clinical examination, including the iliac crest point test, skin-rolling test, and assessment of spinal tenderness. Manual therapy and manipulation are highly effective for treatment; in resistant cases, facet joint injections or radiofrequency applications are preferred. Exercise and posture training to restore proper biomechanics are integral components of the treatment process.
