Odontoid Fraktürler
Özet
Üst servikal omurga kırıklarının %15-20'sini oluşturan odontoid fraktürleri, genellikle yüksek enerjili travmalar veya ileri yaşlarda osteoporoz kaynaklı düşük enerjili düşmeler neticesinde meydana gelir. Stabil kırıklar konservatif yöntemlerle takip edilebilirken, instabil tip 2 kırıklar için hastanın yaşına ve anatomik yapısına göre anterior vida fiksasyonu veya posterior füzyon cerrahisi tercih edilir. Özellikle C2 pedikülünün ince olduğu veya vertebral arterin yüksek seyrettiği riskli vakalarda, C1 lateral kitle ve C2-3 transfaset vidalama tekniği güvenli ve etkili bir cerrahi alternatif sunar.
Odontoid fractures account for 15-20% of all cervical spine fractures and typically result from high-energy trauma in younger patients or low-energy falls related to osteoporosis in the elderly. While stable fractures are managed conservatively, unstable Type 2 fractures require either anterior screw fixation or posterior fusion surgery depending on the patient's age and anatomical characteristics. In complex cases involving thin C2 pedicles or high-riding vertebral arteries, the C1 lateral mass and C2-3 transfaset screwing technique provides a safe and effective surgical alternative.
