Ankilozan Spondilit ve Travma
Özet
56 yaşındaki bir Ankilozan Spondilit hastasında yüksekten cisim düşmesi sonucu oluşan C6-C7 fraktür dislokasyonu, nörolojik kayıp olmaksızın başarılı bir cerrahi müdahale ile tedavi edilmiştir. Ameliyat sürecinde önce posterior yaklaşımla laminektomi ve pedikül vidalarıyla stabilizasyon uygulanmış, ardından anterior plak-vida fiksasyonu ile omurganın mekanik dengesi yeniden sağlanmıştır. Uzman görüşleri, bu tür hastalarda kırılgan kemik yapısı ve zorlu anatomi nedeniyle çok seviyeli enstrümantasyonun hayati önem taşıdığını vurgulayarak uygulanan kombine cerrahi yöntemin etkinliğini doğrulamıştır.
A 56-year-old patient with a 20-year history of Ankylosing Spondylitis was successfully treated for a C6-C7 fracture-dislocation caused by local trauma, without any neurological deficits. The surgical management involved a combined approach, starting with posterior stabilization using pedicle screws and laminectomy, followed by anterior plate-screw fixation to ensure complete spinal stability. Expert evaluations confirm that long-segment instrumentation and combined stabilization are essential in such cases due to increased bone fragility and difficult anatomical positioning inherent to the disease.
