Servikal İntradural Tümör: Nöromonitor ve Pozisyon
Özet
51 yaşında el güçsüzlüğü ve yürüme bozukluğu yaşayan kadın hastanın C6 seviyesindeki intradural kitlesine, başlangıçta posterior yaklaşım ve prone pozisyonu ile müdahale edilmesi kararlaştırılmıştır. Ancak intraoperatif nöromonitorizasyon (IONM) eşliğinde hasta prone pozisyona alındığında, sinyallerde belirgin bir kayıp yaşanması üzerine cerrahi yaklaşım değiştirilerek anterior korpektomi ve kitle eksizyonuna yönelinmiştir. Bu vaka, özellikle servikal omurga cerrahisinde pozisyonel değişikliklerin nörolojik sinyaller üzerindeki etkisinin titizlikle takip edilmesinin ve IONM verilerine göre cerrahi stratejinin revize edilmesinin kritik önemini vurgulamaktadır. Müdahale sonrası nörolojik durumu stabil seyreden hasta, herhangi bir ek sorun yaşamadan taburcu edilmiştir.
A 51-year-old female patient presented with hand weakness and gait disturbance due to a 16x12mm intradural mass at the C6 level. Although a posterior approach in the prone position was initially planned, the surgery was redirected to an anterior corpectomy after intraoperative neuromonitoring (IONM) detected a significant loss of signals upon prone positioning. This case highlights the vital importance of monitoring IONM signals during positional changes in cervical spine surgeries to prevent irreversible neurological damage. Following the successful anterior excision, the patient’s neurological status remained stable, and she was discharged without complications.
