Çoklu Servikal Disk Hernisi
Özet
57 yaşındaki erkek hasta, boyun ağrısı ve dört ekstremitede ilerleyici güç kaybı şikayetiyle başvurmuş; yapılan incelemelerde C4-7 seviyelerinde çoklu disk hernisi ve ciddi spinal kanal darlığı saptanmıştır. İlk olarak uygulanan anterior mikrodiskektomi operasyonu sonrası hastada ani bir felç (parapleji) gelişmesi üzerine acilen ikinci bir müdahale ile posterior laminoplasti uygulanmıştır. Yoğun fizik tedavi sürecinin ardından hasta, postoperatif üçüncü ayda yeniden desteksiz yürüyebilir duruma gelmiştir. Dar kanal zemininde gelişen disk hernilerinde, cerrahi yaklaşım belirlenirken kord basısı riskine karşı çok dikkatli olunması ve gerekirse öncelikle posterior dekompresyonun tercih edilmesi hayati önem taşımaktadır.
A 57-year-old male patient presented with neck pain and progressive weakness in all limbs, with imaging revealing multiple disc herniations at C4-7 and severe spinal stenosis. Following an initial anterior microdiscectomy, the patient developed sudden paraplegia, necessitating an emergency second procedure involving posterior laminoplasty for decompression. After intensive physical therapy, the patient regained the ability to mobilize without support by the third postoperative month. In cases of disc herniation coupled with a narrow spinal canal, surgical strategies must prioritize minimizing cord compression risks, often favoring posterior laminoplasty as an initial step to prevent neurological deficits.
