Kronik Psikiyatrik Hastalıklarda Nörotransmitter Sistemleri
Özet
Bu metin, kronik psikiyatrik hastalıkların nörobiyolojisinde rol oynayan dopaminerjik, glutamaterjik, serotonerjik, GABAerjik, kolinerjik ve adrenerjik nörotransmitter sistemlerinin sentez, metabolizma, reseptör yapıları ve yolaklarını detaylı şekilde incelemektedir. Dopaminerjik sistemdeki bozukluklar şizofrenideki pozitif/negatif belirtiler, DEHB'deki yürütücü işlev eksiklikleri ve ödül mekanizması üzerinden bağımlılıkla ilişkilendirilirken; glutamaterjik sistemdeki NMDA reseptör hipofonksiyonunun şizofreni, majör depresyon ve otizm fizyopatolojisinde kritik rol oynadığı belirtilmektedir. Santral sinir sisteminin ana inhibitör mekanizması olan GABAerjik sistem ile yaygın projeksiyonlara sahip serotonerjik sistemdeki düzensizliklerin anksiyete, depresyon ve alkol bağımlılığına zemin hazırladığı bulgulanmıştır. Kolinerjik sistemdeki nöronal dejenerasyonun Alzheimer hastalığındaki bilişsel düşüşün temel nedenlerinden biri olduğu, adrenerjik sistemdeki locus coeruleus kaynaklı aşırı uyarılmanın ise anksiyete semptomları ile DEHB fizyopatolojisinde etkin olduğu gösterilerek tüm bu sistemlerin psikofarmakolojik tedavilerde temel hedef olduğu vurgulanmaktadır.
This text thoroughly examines the synthesis, metabolism, receptor structures, and pathways of dopaminergic, glutamatergic, serotonergic, GABAergic, cholinergic, and adrenergic neurotransmitter systems playing roles in the neurobiology of chronic psychiatric disorders. Disregulations in the dopaminergic system are associated with positive/negative symptoms in schizophrenia, executive function deficits in ADHD, and addiction via the reward mechanism, while NMDA receptor hypofunction within the glutamatergic system is indicated to play a critical role in the pathophysiology of schizophrenia, major depression, and autism. It is established that abnormalities in the GABAergic system, the primary inhibitory mechanism of the central nervous system, and the widely projecting serotonergic system pave the way for anxiety, depression, and alcohol dependence. It is emphasized that neuronal degeneration in the cholinergic system is a fundamental cause of cognitive decline in Alzheimer's disease, whereas hyperarousal originating from the locus coeruleus in the adrenergic system is active in anxiety symptoms and ADHD pathophysiology, rendering all these systems core targets in psychopharmacological treatments.
