Özet

Kronik psikiyatrik hastalıkların patofizyolojisi, nöral devreler ile reseptör ve reseptör sonrası sinyalleme mekanizmalarının anlaşılmasıyla daha belirgin hale gelmektedir. Şizofreninin pozitif belirtileri mezolimbik dopamin yolağındaki hiperaktivite ve D2 reseptörlerine dopamin bağlanmasıyla ilişkilendirilirken; negatif ve bilişsel semptomlar mezokortikal yolağın hipoaktivitesi ve D1 reseptör yoğunluğundaki azalmayla açıklanmaktadır. Serotonerjik sistemde, 5HT2A ve 5HT1A reseptör etkileşimleri antipsikotik etkinliği belirleyip yan etkileri azaltmaktadır. Noradrenerjik sistemde alfa2 reseptör aktivasyonu prefrontal bilişsel işlevleri iyileştirirken, glutamaterjik sistemde NMDA reseptör hipofonksiyonu nörogelişimsel anomalilere ve dopamin salınımında dengesizliğe yol açmaktadır. Ayrıca, şizofreni hastalarının hipokampusunda östrojen reseptörü alfa mRNA düzeylerinin düşük olduğu saptanmıştır. Duygudurum bozukluklarında ise noradrenerjik, serotonerjik ve dopaminerjik reseptörlerdeki değişimler kritiktir; antidepresanlar alfa ve beta reseptör yoğunluklarını etkilemekte, lityum ise muskarinik reseptör aşırı duyarlılığını bloke etmektedir. Depresyonda 5HT2A ve 5HT2C reseptör sayılarında artış görülürken, nöropeptid Y düzeylerinde azalma, kadınlarda ise östrojen dalgalanmalarının semptom şiddetiyle ilişkisi tespit edilmiştir. Sonuç olarak, psikotrop ilaçların etki mekanizmalarındaki bireysel farklılıkların ve reseptör düzeyindeki etkileşimlerin netleştirilmesi, psikiyatride kişiselleştirilmiş tedavi süreçlerinin, erken teşhisin ve doğru terapötik doz yönetiminin geliştirilmesine stratejik bir katkı sunacaktır.

The pathophysiology of chronic psychiatric disorders is becoming clearer through the understanding of neural circuits and receptor/post-receptor signaling mechanisms. While the positive symptoms of schizophrenia are linked to hyperactivity in the mesolimbic dopamine pathway and dopamine binding to D2 receptors, negative and cognitive symptoms are explained by hypoactivity in the mesocortical pathway and a decrease in D1 receptor density. In the serotonergic system, 5HT2A and 5HT1A receptor interactions determine antipsychotic efficacy and reduce side effects. In the noradrenergic system, alpha2 receptor activation improves prefrontal cognitive functions, whereas NMDA receptor hypofunction in the glutamatergic system leads to neurodevelopmental abnormalities and imbalances in dopamine release. Additionally, low estrogen receptor alpha mRNA levels have been reported in the hippocampus of schizophrenia patients. In mood disorders, changes in noradrenergic, serotonergic, and dopaminergic receptors are critical; antidepressants alter alpha and beta receptor densities, while lithium blocks muscarinic receptor hypersensitivity. Increased numbers of 5HT2A and 5HT2C receptors in depression, decreased neuropeptide Y levels, and the association of estrogen fluctuations with symptom severity in women have been identified. Consequently, clarifying individual differences in psychotropic drug mechanisms and receptor-level interactions will strategically contribute to the development of personalized treatment, early diagnosis, and accurate therapeutic dose management in psychiatry.
