Özet

Bu metin, kronik psikiyatrik hastalıklarda (şizofreni, bipolar bozukluk, depresyon, obsesif kompulsif bozukluk ve anksiyete bozuklukları) nörogörüntüleme yöntemlerinin (MRG, fMRG, BT, PET, SPECT, MRS, DTG) kullanımını ve bu hastalıklara bağlı olarak beyinde meydana gelen yapısal ve işlevsel değişiklikleri incelemektedir. Araştırmalar; şizofrenide ventriküler genişleme, sol hemisferde temporal lob hacim azalması ile dopamin ve GABA seviyelerindeki anomalileri ortaya koymaktadır. Bipolar bozuklukta duygu düzenleme sisteminde artmış, bilişsel kontrol merkezlerinde ise azalmış aktivite saptanırken, lateral ventriküllerde belirgin genişleme ve beyaz cevher bağlantısallığında hasarlar bildirilmiştir. Majör depresyonda prefrontal ve limbik bölgelerde hacim kayıpları ile serotonerjik sistemde fonksiyonel bozulmalar öne çıkmaktadır. OKB'de kortiko-striato-talamo-kortikal devredeki işlev bozuklukları ve kaudat çekirdekteki hacim değişiklikleri dikkat çekerken, bu anormalliklerin tedaviyle geri döndürülebildiği gözlenmiştir. Anksiyete bozukluklarında ve özellikle TSSB'de ise "korku ağı" olarak tanımlanan amigdala, insula ve ön singulat kortekste hiperaktivite, frontal bölgelerde ise hipoaktivite saptanmıştır. Sonuç olarak nörogörüntüleme teknikleri henüz doğrudan bir tanı aracı olmasa da klinik takibi iyileştirme, tedavi başarısını öngörme ve yeni tedavi stratejileri geliştirme konusunda paha biçilemez katkılar sunmaktadır.

This text examines the use of neuroimaging methods (MRI, fMRI, CT, PET, SPECT, MRS, DTI) in chronic psychiatric disorders (schizophrenia, bipolar disorder, depression, obsessive-compulsive disorder, and anxiety disorders) and the structural and functional brain changes associated with these illnesses. Research reveals ventricular enlargement, temporal lobe volume reduction in the left hemisphere, and anomalies in dopamine and GABA levels in schizophrenia. In bipolar disorder, increased activity is found in the emotional regulation system and decreased activity in cognitive control centers, along with prominent lateral ventricular enlargement and white matter connectivity damage. Major depression is characterized by volume loss in prefrontal and limbic regions, as well as functional impairments in the serotonergic system. In OCD, dysfunctions in the cortico-striato-thalamo-cortical circuit and volume changes in the caudate nucleus stand out, and it has been observed that these abnormalities can be reversed with treatment. In anxiety disorders, particularly PTSD, hyperactivity is detected in the "fear network" consisting of the amygdala, insula, and anterior cingulate cortex, alongside hypoactivity in frontal regions. Consequently, although neuroimaging techniques are not yet direct diagnostic tools, they provide invaluable contributions to improving clinical follow-up, predicting treatment success, and developing new therapeutic strategies.
