Özet
Antidepresanlar, dünya çapında işlev kaybının önde gelen nedenlerinden biri olan depresyonun yanı sıra anksiyete, panik atak ve obsesif kompulsif bozukluk gibi rahatsızlıkların tedavisinde ilk basamak seçenekleridir. Bu ilaçlar serotonin, noradrenalin ve dopamin gibi monoaminlerin iletimini artırarak etki gösterirler. En yaygın kullanılanlar seçici serotonin geri alım inhibitörleri (SSRI) ve serotonin-noradrenalin geri alım inhibitörleridir (SNRI). Hastaların yaklaşık %60'ı hafif ila şiddetli yan etkiler yaşamakta ve bu durum tedaviyi bırakma konusunda önemli bir bariyer oluşturmaktadır. Yaygın fiziksel ve gastrointestinal yan etkiler arasında baş ağrısı, mide bulantısı, kusma, uyku bozuklukları, terleme, kilo alımı ve cinsel işlev bozuklukları yer alır. Ayrıca, bazı hastalarda kardiyovasküler etkiler (QT aralığı uzaması, kan basıncı artışı), ilaca bağlı karaciğer toksisitesi, kemik mineral yoğunluğunda azalma ve artmış kanama riski gibi daha ciddi komplikasyonlar gözlenebilir. Genç yaş gruplarında intihar eğiliminin izlenmesi gerekirken, ilaçların aniden veya kademeli olarak bırakılması durumunda grip benzeri semptomlar, titreme ve anksiyete içeren geri çekilme sendromu ortaya çıkabilmektedir. Özetle, yeni nesil antidepresanlar eski tiplere göre daha güvenli olsalar da geniş bir yan etki profiline sahiptirler ve idame tedavisinde nüks riskini azalttıkları için hastaların olası etkiler hakkında eğitilmeleri tedavi uyumu açısından kritik öneme sahiptir.
Antidepressants are first-line treatment options for depression, which is a leading cause of disability worldwide, as well as for anxiety, panic attacks, and obsessive-compulsive disorder. These medications exert their effects by increasing the transmission of monoamines such as serotonin, noradrenaline, and dopamine. The most commonly used types are selective serotonin reuptake inhibitors (SSRIs) and serotonin-noradrenaline reuptake inhibitors (SNRIs). Approximately 60% of patients experience mild to severe side effects, which creates a significant barrier to discontinuing treatment. Common physical and gastrointestinal side effects include headache, nausea, vomiting, sleep disturbances, sweating, weight gain, and sexual dysfunction. Furthermore, more serious complications such as cardiovascular effects (QT interval prolongation, increased blood pressure), drug-induced liver toxicity, decreased bone mineral density, and increased risk of bleeding can be observed in some patients. While suicidal tendencies must be monitored in younger populations, discontinuation syndrome, which includes flu-like symptoms, tremors, and anxiety, may occur upon abrupt or gradual cessation of the medication. In conclusion, although next-generation antidepressants possess a better safety profile than older types, they have a wide range of side effects; since they reduce the risk of relapse during maintenance treatment, educating patients about potential effects is of critical importance for treatment adherence.
