Özet

Bipolar bozukluk, mani, hipomani ve depresif ataklarla seyreden ömür boyu süren yeti yitimi riskli bir durumdur. Tedavide yaygın olarak kullanılan duygudurum düzenleyiciler (lityum, karbamazepin, lamotrijin ve valproat), etkinliklerinin yanı sıra çocuk ve ergen yaş gruplarında sınırlı çalışmalara sahiptir. Lityum, bipolar bozukluğun koruyucu ve akut mani tedavisinde altın standart olarak kabul edilmekle birlikte, dar terapötik penceresi nedeniyle periyodik plazma konsantrasyonu takibi ve böbrek/tiroid fonksiyon testleri gerektirmektedir. Karbamazepin, akut manik ve karma ataklarda FDA onaylıdır ancak agranülositoz ve ciddi dermatolojik reaksiyon (SJS, TEN) riskleri taşır. Lamotrijin, bipolar depresyonun idame tedavisinde nüksleri önlemede etkili olup hayatı tehdit eden cilt döküntüleri (SJS/TEN) yönünden yakın klinik izlem gerektirir. Valproat ise akut mani tedavisinde yaygın bir antiepileptik ajan olmasına rağmen hepatotoksisite, trombositopeni ve kilo alımı gibi yan etkilere yol açabilir; ayrıca teratojenik riskleri sebebiyle çocuk doğurma çağındaki kadınlarda kullanımı sınırlıdır. Ergenlik döneminde tedaviye uyum oranının %40’ın altında olması ve ilaçların yan etki profillerinin değişkenliği, klinisyenlerin kanıta dayalı ve hastaya özgü stratejiler geliştirmesini, güçlü bir terapötik ittifak kurmasını zorunlu kılmaktadır.

Bipolar disorder is a lifelong, disabling condition characterized by manic, hypomanic, and depressive episodes. Mood stabilizers commonly used in its management (lithium, carbamazepine, lamotrigine, and valproate) have limited research regarding children and adolescents despite their widespread use. Lithium remains the gold standard for maintenance and acute mania treatment, yet its narrow therapeutic window necessitates periodic plasma level monitoring alongside renal and thyroid function tests. Carbamazepine is FDA-approved for acute manic and mixed episodes, but it poses risks of agranulocytosis and severe dermatological reactions like SJS or TEN. Lamotrigine is effective in preventing relapses during the maintenance phase of bipolar depression, though it requires close clinical observation for life-threatening skin rashes. Valproate, an antiepileptic drug widely utilized for acute mania, can induce adverse effects including hepatotoxicity, thrombocytopenia, and weight gain, and its prescription is restricted in females of childbearing potential due to teratogenic risks. Given that treatment adherence drops below 40% in adolescents and side effect profiles vary significantly, clinicians must formulate evidence-based, patient-specific strategies while establishing a robust therapeutic alliance to optimize patient outcomes.
