Özet

Alkol ve madde kullanım bozuklukları, küresel ölçekte hastalık ve erken ölüm riskini artıran önemli faktörlerdir. Alkol tüketimi; gastrointestinal sistem fonksiyonlarını bozarak, DNA yapısını değiştirerek ve bağışıklığı zayıflatarak başta sindirim sistemi, karaciğer ve meme kanserleri olmak üzere tüm kanser türlerinin riskini yükseltmektedir. Karaciğer sirozu, hepatit, albüminüri ve kronik böbrek hasarı gibi çok sayıda fiziksel rahatsızlık alkol kullanımıyla doğrudan ilişkilidir. Kardiyovasküler alanda ise iskemik kalp hastalıkları, inme, hipertansiyon ve alkolik kardiyomiyopati gelişimini tetiklemektedir. Solunum sisteminde zatürre ve tüberküloz riskini artıran alkol, sinir sisteminde de deliryum tremens, Wernicke ensefalopatisi ve bunama gibi kalıcı hasarlara yol açar. Gebelikte alkol tüketimi teratojenik etkilerle fetal alkol sendromuna, düşük ve zihinsel engellere sebep olur. Metinde alkolün yanı sıra tütünün akciğer hastalıklarına; kafeinin tansiyon ve kolesterol değişimlerine; kannabis, halüsinojen ve uçucu maddelerin ölümcül aritmi, nörolojik hasar ve toksisiteye yol açtığı belirtilmiştir. Opioidlerin enfeksiyon riskini artırdığı, benzodiazepinlerin bilişsel gerileme yaptığı ve kokainin miyokard enfarktüsü riskini yedi kat artırdığı aktarılmıştır. Bu komorbiditelerin erken teşhisi hayati önem taşımaktadır.

Alcohol and substance use disorders are critical factors that increase global disease burden and early mortality risks. Alcohol consumption elevates the risk of all types of cancer, particularly gastrointestinal, liver, and breast cancers, by disrupting gastrointestinal functions, altering DNA structure, and weakening the immune system. Numerous physical conditions, including liver cirrhosis, hepatitis, albuminuria, and chronic kidney disease, are directly associated with alcohol intake. In the cardiovascular domain, it triggers ischemic heart diseases, stroke, hypertension, and alcoholic cardiomyopathy. Alcohol increases the susceptibility to pneumonia and tuberculosis in the respiratory system, while inducing irreversible damage in the nervous system, such as delirium tremens, Wernicke's encephalopathy, and dementia. During pregnancy, alcohol consumption exerts teratogenic effects, leading to fetal alcohol syndrome, miscarriage, and intellectual disabilities. Alongside alcohol, the text notes that tobacco causes lung diseases; caffeine affects blood pressure and cholesterol levels; while cannabis, hallucinogens, and volatile substances lead to fatal arrhythmias, neurological damage, and toxicity. Furthermore, opioids increase infection risks, benzodiazepines cause cognitive decline, and cocaine elevates the risk of myocardial infarction sevenfold. Early screening and diagnosis of these comorbidities are vital for prevention and treatment.
