Alkol Madde Kullanım Bozukluklarında Nöromodülasyon Tedavileri
Özet
Alkol ve madde bağımlılığı, dünya genelinde yüksek morbidite ve mortalite oranlarına sahip, nüksetme riski yüksek önemli bir halk sağlığı sorunudur. Geleneksel farmakolojik ve psikoterapötik yöntemlerin sınırlı etkinliği nedeniyle, sinirsel aktiviteyi elektriksel, manyetik veya cerrahi uyaranlarla düzenleyen nöromodülasyon tedavileri ön plana çıkmıştır. Bu kapsamda kullanılan Tekrarlayan Transkraniyal Manyetik Stimülasyon (rTMS) ve Transkraniyal Doğru Akım Stimülasyonu (tDCS), kafa derisi üzerinden uygulanan non-invaziv yöntemler olup, özellikle prefrontal korteksi hedef alarak nöroplastisite sağlar, alkol, tütün ve kokain aşermesini ile tüketimini önemli ölçüde azaltır. Üçüncü bir yöntem olan Derin Beyin Stimülasyonu (DBS) ise beyindeki ödül merkezini (Nucleus Accumbens) hedef alan invaziv cerrahi bir prosedürdür ve dirençli vakalarda uzun süreli yoksunluk sağlama potansiyeline sahip olsa da enfeksiyon ve kanama gibi ciddi riskler taşır. Çalışmalar, optimum seans ve akım düzeylerinde bu tekniklerin bağımlılık tedavisinde umut verici sonuçlar doğurduğunu gösterse de klinik etkinliklerinin tam olarak pekişmesi için daha fazla ve çeşitli araştırmalarla desteklenmeleri gerekmektedir.
Alcohol and substance addiction is a significant public health problem worldwide with high morbidity and mortality rates, characterized by a high risk of relapse. Due to the limited efficacy of traditional pharmacological and psychotherapeutic methods, neuromodulation treatments that regulate neural activity through electrical, magnetic, or surgical stimuli have come to the fore. Repetitive Transcranial Magnetic Stimulation (rTMS) and Transcranial Direct Current Stimulation (tDCS), used within this scope, are non-invasive methods applied over the scalp that provide neuroplasticity by especially targeting the prefrontal cortex, significantly reducing alcohol, tobacco, and cocaine craving and consumption. Deep Brain Stimulation (DBS), a third method, is an invasive surgical procedure targeting the reward center of the brain (Nucleus Accumbens); although it has the potential to provide long-term abstinence in treatment-resistant cases, it carries serious risks such as infection and hemorrhage. Studies demonstrate that these techniques yield promising results in addiction treatment at optimum session and current levels, yet they need to be supported by further and diverse researches to fully establish their clinical efficacy.
