Önerilen Tedavinin ve Yoğun Bakım Taburculuğunun Reddedilmesi: Olgu Sunumu
Özet
Bu çalışma, Fournier gangrenine ikincil gelişen septik şok nedeniyle yoğun bakım ünitesine yatırılan 53 yaşındaki bir kadın hastanın, tıbbi endikasyonlara rağmen ameliyatı ve sonrasında yoğun bakım ihtiyacı kalmadığı halde taburculuğu reddetmesi üzerine odaklanan bir olgu sunumudur. Hastaya geniş spektrumlu antibiyoterapi ve cerrahi debritman önerilmiş, ancak hasta ameliyatı kabul etmeyerek yazılı onam vermiştir. Yatışının 5. gününde hemodinamik olarak stabil hale gelen hastanın taburculuğu planlanmış; fakat hasta ve yakınları evde bakım yetersizliğini gerekçe göstererek taburculuğu reddetmiştir. Bu süreçte hastada, uzamış yoğun bakım yatışına bağlı olarak hastane kaynaklı pnömoni ve akut atriyal fibrilasyon gelişmiş, uygulanan ek tıbbi tedavilerle enfeksiyon kontrol altına alınarak hasta önerilerle taburcu edilmiştir. Olgu, hastanın özerklik hakkı ile hekimin tıbbi endikasyonu arasındaki çatışmayı ve alternatifsiz bir tedaviyi reddetmenin sınırlarını tartışmaktadır. Sonuç olarak, hastanın özerklik iradesinin sınırlarının sağlık hukuku açısından nerede bittiği sorusu tartışmaya açılmaktadır.
This study presents a case report of a 53-year-old female patient admitted to the intensive care unit due to septic shock secondary to Fournier's gangrene, who refused the recommended surgical intervention and subsequently declined discharge despite no longer requiring intensive care. Broad-spectrum antibiotic therapy and surgical debridement were recommended, but the patient refused the surgery and signed an informed refusal form. On the 5th day of admission, the patient became hemodynamically stable and discharge was planned; however, the patient and their relatives refused discharge, stating they could not provide care at home. During this prolonged stay, the patient developed hospital-acquired pneumonia and acute atrial fibrillation due to extended intensive care hospitalization; following additional medical treatments, the infection was controlled, and the patient was discharged with recommendations. The case discusses the conflict between patient autonomy and the physician's indication, exploring the boundaries of refusing a treatment that practically has no alternative. In conclusion, the study opens a discussion on where the boundaries of a patient's autonomous will should end within the scope of health law.
