Özet

Üst ekstremite cerrahisi ve ağrı yönetiminde kritik bir rol oynayan rejyonal anestezi tekniklerini ele alan bu çalışma, özellikle brakial pleksus bloklarının (BrP) önemini vurgulamaktadır. Supraklavikular ve aksiller seviyelerde uygulanan bu yöntemler, cerrahi süreçte anestezi ve kas gevşemesi sağlamanın yanı sıra operasyon sonrası dönemde güvenilir bir analjezi sunar. Günümüzde ultrason teknolojisinin kullanımı, bu işlemlerin daha öngörülebilir, etkili ve komplikasyon riskinden uzak bir şekilde gerçekleştirilmesine olanak tanımıştır. Başarılı bir blok uygulaması için dermatom, miyotom ve osteotom yapılarının kapsamlı bir anatomik bilgisine ihtiyaç duyulur. Üst ekstremite, büyük oranda C5-T1 spinal sinir köklerinden köken alan brakial pleksus tarafından inerve edilmektedir. C5-C6 birleşerek üst trunkusu, C7 orta trunkusu, C8-T1 ise alt trunkusu oluşturur. Omzun üst kısmı ve kolun medial yüzeyindeki bazı küçük bölgeler dışında kalan tüm alan bu ağın kontrolündedir. Klinik uygulamalarda anatomik işaretlerin doğru belirlenmesi, blok başarısını doğrudan etkiler. Modern teknikler sayesinde sinirlerin hassas bir şekilde hedeflenmesi, hem hasta konforunu artırmakta hem de müdahale güvenliğini maksimize etmektedir.

This study, which covers regional anesthesia techniques playing a critical role in upper extremity surgery and pain management, emphasizes the importance of brachial plexus blocks (BrP). These methods, applied at supraclavicular and axillary levels, provide anesthesia and muscle relaxation during the surgical process, as well as reliable analgesia in the postoperative period. Today, the use of ultrasound technology has enabled these procedures to be performed in a more predictable, effective manner, and away from the risk of complications. Successful block application requires comprehensive anatomical knowledge of dermatome, myotome, and osteotome structures. The upper extremity is primarily innervated by the brachial plexus, originating from the C5-T1 spinal nerve roots. C5-C6 combine to form the superior trunk, C7 continues as the middle trunk, and C8-T1 combine to form the inferior trunk. The entire area, except for the upper part of the shoulder and some small regions on the medial surface of the arm, is under the control of this network. In clinical applications, the accurate determination of anatomical landmarks directly affects the success of the block. Thanks to modern techniques, precise targeting of nerves both increases patient comfort and maximizes intervention safety.
