Özet
Pediatrik rejyonal anestezi, modern pediatrik anestezi uygulamalarının vazgeçilmez bir parçası haline gelmiştir. Bu teknik, özellikle opioid kullanımının neden olduğu mide bulantısı ve kusma gibi yan etkilerden kaçınarak opioid içermeyen bir süreç yönetimine olanak tanır. Ayrıca, ameliyat sonrası kalıcı ağrı (PPSP) insidansını azaltması ve genel anestezik ajan gereksinimini düşürerek deliryum riskini ve nörotoksisite ihtimalini en aza indirmesi önemli avantajlarındandır. Küçük çocuklarda ağrı eşiğinin düşük olması ve inen inhibitör yolakların yetersiz gelişimi nedeniyle etkili rejyonal analjezi kritik öneme sahiptir. Yapılan geniş çaplı araştırmalar bu yöntemlerin düşük komplikasyon oranlarına sahip olduğunu kanıtlamaktadır. Günümüzde ultrason rehberliği (USR) kullanımı giderek daha fazla önerilmekte; rektus kılıfı, femoral sinir ve siyatik sinir blokları gibi birçok teknikte damarsal yapıların tespiti ve güvenli uygulama için tercih edilmektedir. Çalışmalar, USR'nin tüm rejyonal tekniklerdeki kullanım oranının %23 seviyelerinde olduğunu ve bu yöntemin doğruluğu artırdığını vurgulamaktadır.
Pediatric regional anesthesia has become an integral part of modern pediatric anesthesia practice. This technique allows for an opioid-free anesthesia management, avoiding problematic side effects such as nausea and vomiting. Additionally, it offers significant advantages such as reducing the incidence of persistent post-surgical pain (PPSP) and minimizing the risk of delirium and potential neurotoxicity by decreasing the requirement for general anesthetic agents. Effective regional analgesia is critical in young children due to their lower pain thresholds and underdeveloped descending inhibitory pathways. Large-scale studies demonstrate that these methods have low complication rates. Today, the use of ultrasound guidance (USG) is increasingly recommended and preferred for identifying vascular structures and ensuring safe application in many techniques, such as rectus sheath, femoral nerve, and sciatic nerve blocks. Research highlights that the usage rate of USG in all regional techniques is around 23% and emphasizes that this method enhances accuracy.
