Antenatal Hidronefrozla Başvuran 15 Günlük Erkek Olgu

Özet

Antenatal hidronefroz (ANH) nedeniyle çocuk nefroloji polikliniğine başvuran 15 günlük erkek hastanın, yapılan postnatal renal ultrasonografisinde (USG) sol böbreğinde evre 3 hidronefroz saptanmıştır. Çocuk Nefroloji Derneği CAKUT kılavuzuna göre hastaya amoksisilin profilaksisi başlanmış ve birinci ay kontrolünde de hidronefrozun devam ettiği görülmüştür. Yapılan işeme sistoüretrografisinde (İSUG) vezikoüreteral reflü (VUR) saptanmamış, ancak işlem sonrasında pyelonefrit geçiren hastanın profilaktik tedavisine devam edilmiştir. İleri inceleme için çekilen MAG-3 böbrek sintigrafisinde sol böbrekte inkomplet obstrüksiyon saptanarak hastaya üreterovezikal darlık (UVD) tanısı konulmuştur. Takibinde bir kez daha idrar yolu enfeksiyonu (İYE) geçiren hastanın profilaktik antibiyotiği trimetoprim-sulfametaksazol ile değiştirilmiştir. Böbrek fonksiyon testlerinin normal olması, parankimal kayıp yaşanmaması ve yeni enfeksiyon gelişmemesi üzerine hastanın cerrahi müdahale yerine nonoperatif izlenmesine karar verilmiştir. Şu an 11 aylık olan olgunun takibine böbrek fonksiyon testleri, USG ve idrar kültürü kontrolleriyle devam edilmektedir. Bu tür olgularda ilk iki yılda spontan gerileme olasılığı yüksek olduğundan nonoperatif izlem ön planda tutulmaktadır.

A 15-day-old male infant presented with antenatal hydronephrosis (ANH), and postnatal renal ultrasonography (USG) revealed stage 3 hydronephrosis in the left kidney. Amoxicillin prophylaxis was initiated according to the Pediatric Nephrology Association CAKUT guidelines, and the hydronephrosis persisted during the first-month follow-up. Although voiding cystourethrography (VCUG) showed no vesicoureteral reflux (VUR), the patient developed pyelonephritis after the procedure, and prophylactic treatment was continued. Further evaluation with a MAG-3 renal scintigraphy demonstrated incomplete obstruction in the left kidney, leading to a diagnosis of ureterovesical junction obstruction (UVJO). Following another urinary tract infection (UTI) during follow-up, the prophylaxis was switched to trimethoprim-sulfamethoxazole. Due to normal kidney function tests, the absence of parenchymal loss, and no further infections, nonoperative management was preferred over surgery. The patient, currently 11 months old, is being monitored through renal function tests, USG, and urine cultures. Nonoperative monitoring is prioritized in such cases as spontaneous resolution is highly possible within the first two years of life.
