Antenatal Hidronefrozla Başvuran 3 Günlük Erkek Olgu
Özet
Bu vaka sunumunda, antenatal dönemde bilateral hidronefroz saptanan ve postnatal üçüncü günde çocuk nefroloji kliniğine başvuran erkek bir bebek ele alınmaktadır. Postnatal ilk günlerde yapılan ultrasonografide sol böbrekte ağır (SFU Evre 4), sağ böbrekte ise hafif (SFU Evre 2) hidronefroz gözlenmiştir. İdrar yolu enfeksiyonunu önlemek amacıyla hastaya profilaktik amoksisilin tedavisi başlanmış ve takibe alınmıştır. Postnatal 34. gündeki kontrolde bilateral hidronefrozun kötüleşmesi ve mesane boyutunun artması üzerine çekilen işeme sistoüretrografisinde (İSUG) sağ böbrekte evre 3, sol böbrekte ise evre 4 vezikoüreteral reflü (VUR) tespit edilmiştir. Takipte sekonder VUR nedenleri ekarte edilmiş ve 4. ayda çekilen DMSA sintigrafisinde sol böbreğin diferansiye fonksiyonu %8, sağ böbreğin ise %92 olarak saptanmıştır. Yaşamın erken döneminde ortaya çıkan bu ciddi fonksiyon kaybı, geçirilmiş idrar yolu enfeksiyonu öyküsü de olmaması nedeniyle antenatal dönemdeki intrauterine etkilenmeye bağlı konjenital renal displazi olarak yorumlanmıştır. Hastanın tedavisinde afonksiyonel sol böbrek için cerrahi onarım imkanı bulunmadığından, skar gelişimi olmayan ve enfeksiyon geçirmeyen sağ böbreğin korunması ile VUR'un spontan gerileme olasılığı göz önünde bulundurularak yakın takip kararı alınmıştır.
In this case report, a male infant with bilaterally detected antenatal hydronephrosis who presented to the pediatric nephrology clinic on his third postnatal day is discussed. Ultrasonography performed in the early postnatal days revealed severe hydronephrosis in the left kidney (SFU Grade 4) and mild hydronephrosis in the right kidney (SFU Grade 2). To prevent urinary tract infections, prophylactic amoxicillin treatment was initiated, and the patient was monitored. Upon the worsening of bilateral hydronephrosis and increased bladder size at the postnatal 34-day follow-up, a voiding cystourethrography (VCUG) was performed, which identified grade 3 vesicoureteral reflux (VUR) in the right kidney and grade 4 VUR in the left kidney. Secondary causes of VUR were ruled out during follow-up, and a DMSA scintigraphy performed at the 4th month showed the differential function of the left kidney as 8% and the right kidney as 92%. This severe functional loss presenting early in life, combined with no history of prior urinary tract infections, was interpreted as congenital renal dysplasia due to intrauterine impairment during the antenatal period. Since surgical repair is not feasible for the non-functional left kidney, a close follow-up strategy was decided upon, considering the spontaneous resolution potential of VUR and the preservation of the right kidney, which has no scar formation or infection.
