Yanık Metabolizması ve Fizyopatolojisi
Özet
Yanık, ısı, elektrik, kimyasal maddeler ve radyasyon nedeniyle özellikle yaşlı ve çocuklarda deri ve altı dokularda meydana gelen, tüm organizmayı etkileyen sistemik bir travmadır. Yanık alanında koagülasyon nekrozu gelişerek doku patofizyolojisi merkezi koagülasyon, staz ve hiperemi olmak üzere üç bölgede incelenir. Akut inflamasyon sürecinde vazodilatasyon ve mikrovasküler geçirgenlik artışı gözlenirken, histamin, sitokinler (TNF, IL-1, IL-6) ve nötrofiller gibi birçok mediatör aktif rol oynar; bu süreç sistemik olarak ateş ve lökositoz gibi yanıtlara yol açar. Yanıklar, epidermisin etkilendiği 1. derece, bül oluşumuyla karakterize yüzeyel ve derin 2. derece, epidermis, dermis ve subkutan dokunun tamamen hasarlandığı ağrısız 3. derece ve kas, tendon, kemik gibi derin dokuların yandığı 4. derece olmak üzere derinliklerine göre sınıflandırılır. Yüzeyel yanıklar re-epitelizasyonla iyileşirken, derin yanıklarda makrofaj ve myofibroblastların rol oynadığı granülasyon dokusu ve konnektif doku modülasyonu ile skar dokusu oluşur; iyileşme sürecinde Tip III kollajen zamanla Tip I'e dönüşerek yara 6 hafta sonra eski gücünün %95'ini kazanır. Yanık yarasının onarım başarısı; lokal olarak kan akımı, enfeksiyon ve nekrotik doku varlığından, sistemik olarak ise yaş, beslenme, protein malnütrisyonu ve diyabet gibi faktörlerden doğrudan etkilenir.
Burns are systemic traumas affecting the entire organism, occurring primarily in the elderly and children due to heat, electricity, chemicals, and radiation, causing tissue damage in the skin and subcutaneous layers. Coagulation necrosis develops in the burn area, and tissue pathophysiology is examined in three zones: the central coagulation zone, stasis zone, and hyperemia zone. During the acute inflammation process, vasodilation and increased microvascular permeability are observed, while numerous mediators such as histamine, cytokines (TNF, IL-1, IL-6), and neutrophils play active roles, leading to systemic responses like fever and leukocytosis. Burns are classified by depth into first-degree affecting only the epidermis, second-degree characterized by blister formation (superficial and deep), painless third-degree with total destruction of the epidermis, dermis, and subcutaneous tissues, and fourth-degree involving deeper tissues like muscles, tendons, and bones. While superficial burns heal via re-epithelialization, deeper burns result in scar tissue formation through granulation tissue and connective tissue modulation driven by macrophages and myofibroblasts; during maturation, Type III collagen converts to Type I, allowing the wound to regain 95% of its initial strength after approximately 6 weeks. The success of burn wound healing is directly influenced by local factors such as tissue blood flow, infection, and necrotic tissue, as well as systemic factors including age, nutrition, protein malnutrition, and diabetes.
