Yanık Hastalarında İmmün Sistem ve Enfeksiyonları

Özet

Yanık travması, lokal ve sistemik immün sistemde derin fonksiyon bozukluklarına yol açarak doğal ve kazanılmış bağışıklık cevaplarını baskılamakta, bu durum ise yara iyileşmesini geciktirip enfeksiyon riskini ve sepsis kaynaklı mortaliteyi büyük ölçüde artırmaktadır. Travma sonrasında monosit, makrofaj, nötrofil ve doğal öldürücü hücrelerin aktiviteleri ile kompleman sistemi dengesi bozulurken; proinflamatuar sitokinler (TNF-ɑ, IL-1B, IL-6, IL-8) ile antiinflamatuar sitokinler (IL-10) aynı anda salınarak hiperinflamasyon ve immün depresyona yol açar. Hücresel düzeyde Th-1/Th-2 oranı ile CD4+/CD8+ oranları azalırken, Treg hücrelerinin artışı T efektör hücrelerini baskılar; hasarlı dokulardan salınan DAMP ve PAMP molekülleri ise patern tanıyıcı reseptörler üzerinden sistemik enflamatuar cevap sendromunu (SIRS) tetikler. Yanık alanında mikroorganizmaların birikmesiyle önce yara yeri kolonizasyonu ve biofilm tabakası oluşmakta, süreç zamanla invaziv enfeksiyona ilerlemektedir. Küresel ölçekte en yaygın invaziv enfeksiyon etkeni metisiline dirençli Staphylococcus aureus (MRSA) iken, karakteristik sarı/yeşil akıntıyla seyreden Pseudomonas ve fırsatçı Acinetobacter türleri de sepsise zemin hazırlar. Solunum sistemi tutulumu, kateter ilişkili tromboflebit ve bozulan bağırsak mikrobiyotasından kaynaklanan bakteriyel translokasyon, yanık hastalarında çoklu organ yetmezliğinin ve septik ölümlerin başlıca nedenleridir.

Burn injury leads to profound dysfunctions in both local and systemic immune systems, suppressing innate and adaptive immune responses, which in turn delays wound healing and significantly increases infection risks and sepsis-related mortality. Following trauma, activities of monocytes, macrophages, neutrophils, and natural killer cells, as well as the complement system balance, are disrupted; meanwhile, the simultaneous release of proinflammatory cytokines (TNF-ɑ, IL-1B, IL-6, IL-8) and antiinflammatory cytokines (IL-10) triggers hyperinflammation and immune depression. At the cellular level, Th-1/Th-2 and CD4+/CD8+ ratios decrease, whereas the increase in Treg cells suppresses T effector cells; furthermore, DAMP and PAMP molecules released from damaged tissues trigger systemic inflammatory response syndrome (SIRS) via pattern recognition receptors. Microorganism accumulation in the burn site initially causes wound colonization and biofilm formation, which progressively advances into invasive infection. Globally, methicillin-resistant Staphylococcus aureus (MRSA) is the leading cause of invasive infections, while Pseudomonas, presenting with characteristic yellow/green discharge, and opportunistic Acinetobacter species also pave the way for sepsis. Respiratory system involvement, catheter-associated thrombophlebitis, and bacterial translocation due to compromised intestinal microbiota are the primary causes of multiple organ failure and septic deaths in burn patients.
