Uyarıcı Maddeler ve Kullanım Bozuklukları
Özet
Amfetaminler, metamfetamin ve kokain gibi uyarıcı maddelerin tarihçesi, epidemiyolojisi, nörofarmakolojisi, klinik etkileri ve tedavi yöntemleri incelenmektedir. Geleneksel bitkilerden izole edilen ve zamanla sentetik olarak geliştirilen bu maddeler, başlangıçta tıbbi amaçlarla kullanılmış olsa da kötüye kullanım potansiyelleri nedeniyle yasal düzenlemelere tabi tutulmuştur. Dünya genelinde esrardan sonra en yaygın yasa dışı uyuşturucu sınıfını oluşturan uyarıcılar, merkezi ve periferik sinir sistemindeki dopamin, norepinefrin ve serotonin monoaminerjik yolları üzerinden etki ederek öfori, uyanıklık artışı ve hiperaktiviteye yol açar. Kronik kullanımları; paranoya ve halüsinasyon gibi psikotik semptomların yanı sıra inme, enfarktüs ve böbrek yetmezliği gibi ciddi tıbbi durumlara zemin hazırlar. Bilişsel Davranışçı Terapi (BDT) ve acil durum yönetimi gibi psikososyal yaklaşımlar bağımlılık tedavisinin temelini oluştururken, akut intoksikasyon durumlarında benzodiazepinler ilk seçenek olarak tercih edilmektedir. Halihazırda FDA tarafından onaylanmış spesifik bir ilaç bulunmamakla birlikte, naltrekson, bupropion ve disülfiram gibi ajanların kullanımına yönelik klinik araştırmalar ve agonist ikame tedavileri umut vadetmektedir.
The history, epidemiology, neuropharmacology, clinical effects, and treatment methods of stimulant substances such as amphetamines, methamphetamine, and cocaine are examined. These substances, which were isolated from traditional plants and synthetically developed over time, were initially used for medical purposes but were subjected to legal regulations due to their high abuse potential. Formulating the second most common class of illicit drugs worldwide after cannabis, stimulants act through monoaminergic pathways of dopamine, norepinephrine, and serotonin in the central and peripheral nervous systems, leading to euphoria, increased alertness, and hyperactivity. Their chronic use paves the way for severe medical conditions including stroke, myocardial infarction, and kidney failure, alongside psychotic symptoms such as paranoia and hallucinations. Psychosocial approaches like Cognitive Behavioral Therapy (CBT) and contingency management form the cornerstone of addiction treatment, whereas benzodiazepines are preferred as the first-line option in acute intoxication cases. Although there is currently no specific medication approved by the FDA, clinical research regarding the use of agents such as naltrexone, bupropion, and disulfiram, as well as agonist replacement therapies, shows promise.
